Community Health Program Assistant

Purpose:

Provides program administrative and indirect clinical support for Community Health teams.


Responsibilities:

  • Assists team, clients, family members and other contacts according to general performance standards, demonstrating organizational values with all interactions while actively contributing to a positive work environment.
  • Collaborates with Community Health leadership to coordinate volunteer needs and opportunities, including Share the Joy and Perinatal Pack organization.
  • Triages Community Health information requests in collaboration with leadership and Coordinated Intake.
  • Answers intake phone as needed.
  • Prepares and orders supplies, equipment and educational materials for Community Health teams.
  • Maintains client records in collaboration with Community Health Data and Quality Manager.
  • Actively protects HIPAA regulations by ensuring patient information is kept secure and private.
  • Performs timely data entry and billing with accuracy for Community Health teams.
  • Ensures office environment is kept clean and free from clutter.
  • Serves as marketing liaison for material ordering and inventory.
  • Organizes meetings, client events, projects, trainings, and completes meeting minutes.
  • Provides support for social events held in the community and/or EveryStep locations.
  • Serves as a primary resource for the team for office duties such as distribution of mail, making copies, filing, and scheduling team meetings.
  • Coordinates and/or assists with special projects as assigned.

Qualifications:

  • High School diploma or equivalent preferred
  • Quality customer service skills
  • Knowledge of computer software, including but not limited to Microsoft Suite
  • Ability to multi-task between data entry, data support and other clerical duties
  • Time management skills with the ability to prioritize workload and seek clarification when needed
  • Ability to problem-solve and communicate effectively both verbally and in writing
  • Team player who focuses on productivity and results
  • Ability to work among multidisciplinary teams
  • Highly detail oriented with a critical degree of accuracy regarding data entry and proof reading
  • Must have valid driver’s license, auto insurance and reliable transportation if driving is required
  • Successful completion of pre-employment physical, drug screen, 2-step TB test and background check

Working Conditions:

  • Physical requirements: Light work, sitting most of the times, exerting up to 20 pounds of force frequently, and/or small amount of force constantly to move objects
  • Environment: Primarily in a climate-controlled environment
  • Time: General schedule will be in the office from 8-4:30 Monday-Friday with the ability to be flexible with a varying schedule and programmatic requirements
  • Travel: May be required between locations
