ContinuousNature of Work:
Conduct assessments using the interRAI in which nursing skills are used to evaluate the health and psychosocial status of a person who has been referred to Assessment, Evaluation and Review Services (AERS).
Conduct an evaluation in which nursing skills are used. This would include the ability to recommend appropriate services based on the physical and psychosocial assessment, leading to an initial plan of intervention and recommending resources and other programs to benefit the individual. Communicate and follow up as needed with stakeholders, families and clients as appropriate.
Manage the schedule to accommodate competing priorities, allowing for cancellations and client-related delays, while responding to clients' needs and demonstrating the ability to accommodate clients within the program's framework. Respond to clients' calls in a timely manner.
Develop a comprehensive interdisciplinary plan of care, identifying needs and significant findings, recommendations, and a plan of care demonstrating a direct correlation between significant findings. Demonstrate appropriate follow-up with other stakeholders such as the reportable events unit, support planners and nurse monitors.
Present cases in the regular team meetings for input from other assessors. Share resources and experiences with the team to help them make better referrals and recommendations. Attend other assessments for observation and learning as needed.
Complete ancillary and follow-up documentation in a complete and timely manner, which would be reflected in the Long Term Services and Support System (LTSS Maryland), Apricot software and paper file.
Administrative requirements and special projects as assigned; measured by a willingness to participate in special projects and assigned tasks, completion of at least one special project per year, and attendance of at least one seminar per year.
Education:
Associate degree in nursing or related field from an accredited college or university.
Experience:
Two years of experience in a non-hospital setting.
License or Certification:
Registered Nurse.
Notes:
Must be comfortable doing home visits and traveling throughout the county.
Desired or Preferred Qualifications:
Experience in long-term care, home health, community health, Bachelor of Science in Nursing (BSN).
