Job Overview
We are seeking a dedicated and compassionate Community Health Worker to join our team. In this role, you will serve as a vital link between the community and healthcare services, helping individuals navigate health resources and improve their overall well-being. Your work will involve educating and empowering community members, particularly those with disabilities, to make informed health decisions and access necessary services.
Responsibilities
- Conduct outreach to engage community members and assess their health needs.
- Provide education on health topics, including nutrition, public health initiatives, and available resources.
- Assist individuals in navigating healthcare systems, including Medicare and other insurance options.
- Facilitate access to services for people with disabilities and other vulnerable populations.
- Collect data on community health trends and individual health outcomes to inform program development.
- Employ motivational interviewing techniques to encourage healthy behaviors among clients.
- Collaborate with healthcare providers, social workers, and community organizations to coordinate care.
- Deliver presentations and workshops on relevant health topics to enhance community awareness.
Skills
- Strong background in Nursing or Social Work is preferred.
- Experience working with people with disabilities is highly valued.
- Proficiency in Public Health principles and practices.
- Ability to teach effectively and engage diverse audiences through Public Speaking.
- Familiarity with Data Collection methods for health assessments and program evaluation.
- Knowledge of Medical Terminology relevant to patient care and communication.
- Understanding of Medicare processes and eligibility requirements.
- Skills in Nutrition counseling to promote healthy eating habits within the community.
