Duties
- Conduct outreach to diverse populations, including individuals with disabilities and those facing social or health challenges
- Provide health education on topics such as nutrition, addiction counseling, and medication management (including Medicare)
- Use motivational interviewing techniques to encourage positive behavioral changes and improve health habits
- Assist clients with understanding medical terminology and navigating healthcare systems
- Collect and document data related to client interactions, health status, and service needs accurately and confidentially
- Support clients in accessing community resources, including social services, housing assistance, and healthcare programs
- Deliver engaging public speaking sessions to educate groups on health topics and promote wellness initiatives
Experience
- Prior experience or training in nursing, social work, public health, or related fields is highly desirable
- Experience working with individuals with disabilities or in addiction counseling is a strong plus
- Proven ability to communicate effectively through public speaking and one-on-one interactions
- Familiarity with data collection methods and maintaining accurate records in healthcare settings
- Knowledge of medical terminology, Medicare processes, and nutrition principles is advantageous
- Demonstrated skills in teaching, community outreach, or health promotion activities
- Ability to build trust quickly through empathetic listening and motivational interviewing techniques
