Job Summary: Community Health Worker / MNsure Navigator (Bilingual Spanish/English Preferred)

C.A.R.E. Clinic in Red Wing is seeking a compassionate and motivated Community Health Worker (CHW)/MNsure Navigator to support patients in accessing dental care, health insurance, and essential community resources. This dual-role position works closely with the dental team to assist with patient intake, education, and care coordination, while also helping uninsured individuals enroll in health coverage through MNsure.

The CHW serves as a trusted liaison between patients and healthcare/social service systems, helping individuals overcome barriers related to cost, transportation, language, and access to care. Responsibilities include conducting dental intake assessments, determining eligibility for financial assistance programs, assisting with MNsure applications, coordinating appointments and referrals, providing health education, and supporting front desk and clinical operations as needed.

This role is ideal for someone who is passionate about community health, enjoys working with diverse populations, and is committed to improving access to care through outreach, advocacy, and patient support.

Qualifications:

• Bilingual in Spanish and English preferred
• High school diploma or GED required
• Strong interpersonal and communication skills
• Experience navigating healthcare or social service systems preferred
• Basic computer proficiency (Microsoft Office)
• MNsure Navigator Certification (or willingness to obtain upon hire)

Pay: From $22.00 per hour
