Position Summary

We are seeking a dynamic and relationship-driven Community Liaison to join our growing team in Gillette, WY. This role is responsible for developing and maintaining key referral relationships, increasing census growth, and serving as a trusted resource to healthcare providers and the community.

The ideal candidate is a motivated self-starter who thrives in a fast-paced environment and is passionate about connecting patients with the care they need.

Key Responsibilities

  • Develop and manage relationships with physicians, hospitals, case managers, skilled nursing facilities, and other referral sources
  • Drive patient census growth through strategic outreach and territory planning
  • Educate referral partners on Healing Hearts’ services, programs, and clinical capabilities
  • Identify new business opportunities and expand market presence
  • Coordinate with internal clinical teams to ensure seamless patient admissions
  • Track and report referral activity, trends, and outcomes
  • Represent the organization at community events, health fairs, and networking opportunities
  • Maintain compliance with all healthcare regulations and company policies

Qualifications

  • Bachelor’s degree in healthcare, business, marketing, or related field preferred
  • Minimum of 1–2 years of experience in healthcare sales, home health, hospice, or related field strongly preferred
  • Proven ability to build and maintain professional relationships
  • Strong communication, presentation, and organizational skills
  • Self-motivated with the ability to work independently and manage a territory
  • Knowledge of home health services and referral processes preferred
  • Valid driver’s license and reliable transportation
