Community Nurse

Duties:

• Supervises Personal Care Partners/ Nursing staff
• Checks in medications from Pharmacy/ monitors seniors levels of care
• In charge of overseeing MD orders for correctness and update MAR/ Service plan
• Contact with Pharmacy
• Completes assessments yearly and as needed
• Screens new seniors for admission/ works with families and seniors for all nursing needs.
• Monthly weights/ VS of seniors
• Chart as needed / review and update service plans
• TB skin tests to staff and seniors as needed
• Assist in dining room as needed
• Assist seniors with ADLS as needed (Trains PCP to fulfill these duties)
• Respond to nurse call system and any emergencies
• Assist Community Director as needed with activities and events
• On call – schedule supervises nursing needs of building
• Carry a spectra link phone daily for easy communication with other staff.
• All other duties as assigned

Required:

• Current LPN/RN License
• Prior nursing experience
• Previous Care giving experience
• Able to speak and write English
• Must have a current State of Illinois Nursing License
• Able to work independently
• Able to work overtime as needed
• CPR certified
• Food Handler Certificate

Job Type: Full-time

Pay: $28.00 - $35.00 per hour
