Responsibilities
- Conduct thorough patient assessments, including vital signs, medication reconciliation, and health evaluations to develop personalized care plans.
- Administer medications and treatments following established protocols, including injections, IV infusions, and wound care such as suturing and ostomy management.
- Collaborate with interdisciplinary teams to coordinate care, ensuring compliance with healthcare standards such as HIPAA and NCQA standards.
- Provide health education and coaching tailored to individual needs, including memory care, geriatrics, behavioral health, and chronic disease management.
- Monitor for acute pain or emergent health issues, triaging appropriately and facilitating referrals to emergency services or primary care providers when necessary.
- Maintain accurate medical documentation using EMR (Electronic Medical Record) systems like Epic or Cerner; ensure all records are complete and compliant with legal requirements.
- Support community outreach initiatives by working with individuals with developmental disabilities or those in assisted living environments to promote wellness and safety.
Qualifications
- Valid nursing license LVN with current registration in the state of practice.
- Proven experience in long-term care settings such as nursing homes or assisted living facilities.
- Strong clinical skills including IV insertion, medication administration, aseptic technique, wound suturing, and catheter care.
- Familiarity with EMR/EHR systems such as Epic or Cerner; experience with ICD coding (ICD-9/10) is preferred.
- Knowledge of complex healthcare areas including hospice & palliative medicine, trauma medicine (Level I & II trauma centers), infectious disease care, and acute care environments like ICU or emergency room experience.
- Excellent patient assessment skills with the ability to work independently while collaborating effectively within multidisciplinary teams.
