Trusted Path Home Health is seeking a motivated and professional Community Outreach Liaison to represent our agency in the healthcare community. This role is ideal for someone who wants to grow in the healthcare field, enjoys building relationships, and thrives in a performance-based environment.
This is a marketing and appointment-setting position. It is not a patient referral or admissions role.
Your mission is to introduce our agency to healthcare professionals and schedule high-quality meetings with decision makers so our leadership team can present our services.
Primary Objective
Secure an average of 10 qualified meetings per week with healthcare decision makers.
What You’ll Do
- Prospect healthcare organizations such as:

- Hospitals
- Post-acute centers
- Skilled Nursing Facilities
- Doctor offices
- Assisted Living communities

- Professionally introduce our agency and services
- Build long-term professional relationships
- Schedule qualified meetings with decision makers
- Maintain basic tracking of outreach activity
- Represent the company with professionalism and confidence
What You Will Not Do
- You will not solicit patients
- You will not discuss patient cases
- You will not handle admissions or referrals
- You will not be compensated based on patient volume or revenue
Compensation (Commission Only)
- $15 per new verified qualified referral source
- $100 per qualified meeting completed
- No income cap
- Paid strictly on outreach activity and appointment setting
- Compensation is not tied to patients, admissions, or medical billing
Definitions for Clarity
Qualified Referral Source ($15):
A verified healthcare organization that:
- Actively works with patients or discharges patients
- Has an identified department or decision maker (case management, social work, office manager, administrator, etc.)
- Includes a valid contact name, title, and phone/email
- Confirms openness to professional meetings
Payment is made only after the source is reviewed and approved.
Qualified Meeting ($100):
A meeting that:
- Is scheduled in advance
- Is a minimum of 15–20 minutes
- Takes place with a decision maker such as:

- Case manager
- Social worker
- Discharge planner
- Office manager
- Practice administrator
- Physician

- Occurs as scheduled (not canceled or no-show)
Payment is issued only after the meeting is completed and verified.
Cancellations & No-Shows
- If the facility cancels or reschedules, payment is made only after the meeting is completed
- If the liaison cancels or the meeting does not occur, no payment is issued
Optional Performance Bonuses (if you choose to include them later)
- 10 completed meetings in one week → bonus
- 40 completed meetings in one month → bonus
(These are optional and can be added once performance patterns are established.)
