Location: Brooklyn, NY office (primary) — with occasional Bronx field work and the option to work from home as needed

Nutripro is expanding our work through New York's Social Care Network (SCN), a Medicaid initiative that connects eligible individuals and families to essential services, such as nutritious food, housing assistance, transportation, and other health-related social supports. We're looking for a Community Outreach & Social Care Navigator to help members access the resources that can improve their health and quality of life.

What You'll Do

  • Conduct SCN eligibility screenings with Medicaid members, both virtually (phone/video) and in person as needed
  • Guide eligible members through enrollment and navigation, and follow up to ensure their needs are addressed
  • Build relationships with clinics, community-based and faith-based organizations, senior centers, and other partners
  • Represent Nutripro at health fairs, community events, and partner locations
  • Educate individuals and families about the services available to them
  • Complete screenings, assessments, and documentation accurately, and keep timely records in our CRM
  • Track outreach and field activities for program reporting and billing

Who We're Looking For

The most important qualification is a genuine desire to help people. We can teach our programs, systems, and Medicaid navigation. What we can't teach is compassion, initiative, and reliability, so that's what we're hiring for.

You're likely a strong fit if you:

  • Take initiative and get things done without constant supervision
  • Are dependable and follow through on your commitments
  • Build trust quickly with people from diverse backgrounds and are comfortable starting conversations
  • Stay organized and accurate with documentation, even when the day's priorities shift
  • Are comfortable with computers, tablets, smartphones, and CRM systems, and pick up new tools quickly
  • Work well both independently and as part of a team

Required Qualifications

  • Fluency in English, with strong written and verbal communication skills
  • Able to work primarily from our Brooklyn office and travel to the Bronx for screenings, outreach, and events as needed (no driver's license required, as long as you're comfortable getting around the city)
  • Strong computer skills and comfort learning new software

Preferred Qualifications

  • Fluency in a second language commonly spoken in the communities we serve (Spanish strongly preferred; Arabic, Bengali, French, Haitian Creole, and others welcome)
  • Experience in community outreach, social services, care coordination, Medicaid navigation, healthcare, customer service, nonprofit work, education, or a related field
  • Experience working with Medicaid populations, older adults, individuals with chronic health conditions, or underserved communities

Experience helps, but we're willing to train the right person. We'd rather hire someone with the right attitude and work ethic than someone with a long résumé and little passion for the work.

Compensation & Schedule

  • Full-time, W-2 position
  • $22–$25 per hour, based on experience
  • Monday–Friday, 9:00 AM – 5:00 PM, with occasional evening or weekend community events
  • Based primarily at our Brooklyn office, with occasional Bronx field work and the option to work from home when office or field presence isn't required
  • Opportunities for growth as our SCN program expands

Nutripro is a New York City–based meal delivery company serving Medicaid members through the SOMOS Social Care Network. We provide nutritionally appropriate meals that help individuals manage chronic conditions, improve health outcomes, and stay connected to essential support services.
