Community Support Case Manager

We are currently seeking a dedicated and resourceful Part-Time Case Manager to join our interdisciplinary team. The ideal candidate is proactive, well-organized, and has a deep understanding of the barriers faced by underserved communities. This is an opportunity to make a tangible impact in the lives of individuals navigating complex behavioral and social challenges.

Position Summary

The Case Manager will serve as a key point of contact and support for clients enrolled in HealHub’s recovery program. From intake to discharge, the Case Manager will coordinate comprehensive care services, assist with government benefit applications, and connect clients to essential resources such as housing, employment, identification documentation, and long-term support. This role requires strong communication skills, patience, empathy, and the ability to work collaboratively within a clinical team.

Key Responsibilities

Client Onboarding & Orientation

  • Conduct comprehensive intake interviews and program orientations for all new clients.
  • Explain clinic services, client expectations, program structure, and recovery goals.
  • Build rapport and establish trust to ensure client engagement and accountability.

Needs Assessment & Planning

  • Complete biopsychosocial assessments to evaluate client needs and barriers.
  • Develop individualized action plans that address housing, employment, documentation, health care, and financial stability.
  • Maintain up-to-date knowledge of local and federal support programs (e.g., Medicaid, EBT, TANF, Social Security, GAU/ABD).

Resource Navigation & Advocacy

  • Assist clients in obtaining vital documents including state IDs, birth certificates, and Social Security cards.
  • Help clients apply for public assistance programs including disability benefits, food stamps, and rental assistance.
  • Provide referrals and connections to community-based organizations, employment programs, housing agencies, and legal services.

Ongoing Case Management

  • Conduct regular follow-ups and check-ins to monitor progress on goals.
  • Collaborate with clinical staff, therapists, peer counselors, and medical providers to ensure continuity of care.
  • Update client case files and documentation using electronic health record systems (EHR).

Discharge Planning

  • Create and implement discharge plans that promote long-term independence, including stable housing, employment, or educational placement.
  • Ensure warm hand-offs and support during the transition out of the program.
  • Provide post-discharge referrals and aftercare planning when appropriate.

Required Qualifications

  • Associate’s, Bachelor’s, or Master’s Degree in Social Work, Psychology, Human Services, or a related Social Sciences field (Bachelor’s/Master’s in Social Work preferred).
  • Minimum 1 year of direct experience in case management, preferably within behavioral health, substance use, or homelessness services.
  • Familiarity with navigating governmental systems, benefits programs, and housing/employment resources.
  • Experience working with sensitive or confidential information in compliance with HIPAA and clinical documentation standards.
  • Strong written and verbal communication skills with proficiency in behavioral note-taking and documentation.
  • Ability to work independently, manage caseloads effectively, and exercise sound judgment.
  • Must be able to pass a background check and maintain required employment clearances.
  • Reliable transportation and the ability to commute to the Renton, WA clinic.

Preferred Qualifications

  • Bachelor’s or Master’s degree in Social Work (BSW/MSW)
  • Prior experience in a behavioral health clinic, substance use recovery program, or homeless services agency
  • Bilingual in Spanish, Somali, or another frequently spoken language in the community

Schedule & Work Environment

  • Work Schedule: Part-time, Tuesday through Thursday
  • Hours: 8-hour shifts, 24 hours per week
  • Work Location: In-person at HealHub Behavioral Health Clinic in Renton, WA
  • Work Environment: Team-based, trauma-informed, culturally responsive, fast-paced clinical setting

Job Type: Part-time

Pay: $23.00 - $26.00 per hour
