Community Support Specialist - Adult and Youth

Description:

We are currently seeking a dedicated Community Support Specialist to join our compassionate and mission-driven team, supporting our Targeted Case Management (TCM) program in the Western Region (Allegany and Garrett Counties). This vital position supports clients aged 5–17 and 18 and older, including their families or caregivers, by providing comprehensive, community-based case management services.

The Community Support Specialist will provide case management services by identifying client needs, assessing strengths and barriers, and collaborating on individualized service plans to support mental health stabilization, physical well-being, educational engagement, social-emotional development, and economic self-sufficiency. Your work will directly support our mission to expand access, improve outcomes, and empower the communities we serve.

Requirements:

Essential Job Responsibilities:

  • Provide community- and home-based support to clients and caregivers at a frequency aligned with regulatory requirements and individual needs.
  • Conduct assessments and assist in the development and implementation of individualized plans of care.
  • Link clients and families to appropriate community resources, including behavioral health services, medical care, housing, education, and employment supports.
  • Coordinate care across systems by collaborating with multidisciplinary providers to ensure continuity and effectiveness of services.
  • Advocate for client needs with schools, community agencies, and service providers to promote access, stability, and well-being.
  • Maintain accurate, timely documentation in accordance with agency, state, and federal guidelines.
  • Complete ongoing trainings to ensure compliance with agency policies, state regulations, and quality standards for service deliver
  • Transport clients to essential services and appointments using a personal vehicle or company-provided transportation, as appropriate.
  • Perform other duties as assigned to support client outcomes and program effectiveness

Qualifications and Keys for Success:

  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field is required; a master’s degree is preferred.
  • At least 1 year of relevant experience in case management or community-based behavioral health is highly desirable.
  • Familiarity with trauma-informed care and strength-based service delivery models.
  • Strong communication, time management, and organizational skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Valid driver's license and access to reliable transportation required.

EXPERIENCE AND EDUCATION:

REQUIRED CERTIFICATION OR LICENSE:

? CPR and First Aid Certified

PHYSICAL ABILITIES:

? Occasionally lift, carry, push pull or otherwise move objects weighing up to25 pounds

? Regularly drives a motor vehicle.

WORKING CONDITIONS:

Work is performed in various locations throughout the community. Work requires the ability to independently travel to various locations within the areas serviced.
