What You’ll Do

Depending on assignment (CHW or PCS support), you may:

• Support clients in their homes or communities
• Help with daily living support or care coordination
• Connect clients to healthcare and community resources
• Assist with transportation, appointments, and basic needs
• Provide encouragement, check-ins, and support
• Document simple visit notes in an app or system

Who We’re Looking For

We welcome ALL experience levels.

Entry-Level Candidates:

• No experience required
• Must be willing to learn and grow
• Compassionate, dependable, and people-focused

Preferred Qualifications:

• High school diploma or GED
• Willingness to complete CHW certification (state-approved program)
• OR experience as a caregiver, home health aide, CNA, or similar role
• Reliable transportation
• Basic smartphone/computer skills
• Strong communication skills
