We are seeking a dedicated Qualified Professional (QP) to join our Community Support Team (CST). The QP will provide direct and indirect support to adults with mental health and/or substance use challenges, ensuring that services are delivered in accordance with the Person-Centered Plan (PCP) and Medicaid guidelines. This is a billable service role.
Responsibilities:
- Provide CST interventions in the community, home, or other appropriate settings.
- Maintain and document billable service hours per agency and Medicaid requirements.
- Complete assessments, service plans, progress notes, and other documentation accurately and timely.
- Coordinate with clinical and medical staff, natural supports, and other service providers.
- Support individuals in achieving treatment goals related to housing, employment, daily living, and wellness.
- Participate in weekly team meetings, supervision, and case consultations.
Qualifications:
- Bachelor’s degree in Human Services or related field (Social Work, Psychology, Counseling, etc.).
- Minimum of two years of post-degree experience working with the adult mental health or substance use population.
- Knowledge of CST service definitions and North Carolina Medicaid requirements preferred.
- Strong communication, organization, and teamwork skills.
- Must have valid NC driver’s license and reliable transportation.
