About the Community Wellness Health Director Role
As the Community Wellness Health Director within our Community Wellness service line, you will provide strategic and operational leadership for contracted community wellness programs, including payer-aligned initiatives and transition services. This role ensures contractual compliance, completion of required nursing reviews, and adherence to regulatory and accreditation standards, including NCQA requirements.

You will oversee compliance monitoring, data collection, and reporting processes while representing SAM in statewide regulatory meetings and professional forums. This position requires a leader who can balance regulatory precision with relationship-centered partnership management to sustain program integrity and high-quality outcomes.

Community Wellness Health Director Core Responsibilities

  • Provide strategic oversight of the Geisinger Community Health Workers contract to ensure quality, compliance, and performance outcomes.
  • Oversee the Pennsylvania Health and Wellness Community Transition Worker Program in alignment with contractual and regulatory expectations.
  • Complete required payer nursing reviews with accuracy and timeliness.
  • Monitor compliance measures, evaluate performance trends, and implement corrective action plans as needed.
  • Maintain NCQA accreditation standards, including leading data collection efforts and facilitating required meetings.
  • Cultivate and sustain strong internal and external stakeholder relationships to ensure partnership alignment and program stability.
  • Represent the organization in statewide regulatory meetings, payer forums, and relevant professional conferences.

What You Bring

  • Registered Nurse licensure required.
  • Minimum of five (5) years of supervisory or leadership experience, or a combination of relevant experience and education.
  • Demonstrated experience with regulatory compliance, payer contracts, and accreditation standards (NCQA experience strongly preferred).
  • Strong analytical skills with the ability to translate regulatory requirements into operational systems and practice.
  • Executive-level communication skills and the ability to represent the organization with professionalism and authority.
  • Valid driver’s license and reliable transportation.
