Comprehensive Case Manager (Bilingual in English and Spanish)

The Comprehensive Case Manager provides structured, trauma-informed case management and ongoing support for children and families impacted by abuse. This role focuses on stabilization, engagement, and continuity of services following the initial forensic interview, ensuring families remain supported throughout the investigation and healing process. The Comprehensive Case Manager conducts trauma and suicide screenings, develops and implements individualized Family Service Plans, and provides consistent follow-up to address barriers, strengthen caregiver capacity, and support families during critical periods such as therapy waitlists and case progression. This role serves as a central point of support for families while collaborating closely with CAC services and multidisciplinary team partners to promote access, engagement, and overall family stability.

Essential Functions

  • Administer and document trauma and suicide screenings in accordance with CACNT protocols to identify needs and inform service planning.
  • Collaborate with clinical staff to interpret screening results and determine appropriate referrals and next steps.
  • Develop, implement, and maintain individualized Family Service Plans (FSPs) that reflect evolving family needs, strengths, and priorities.
  • Assign and adjust tiered levels of support based on family needs, risk factors, and engagement.
  • Provide ongoing, structured follow-up and stabilization support for families, particularly during therapy waitlist periods and other transitions in services.
  • Monitor and track referrals for therapy, medical services, and community resources; identify and address barriers to access and engagement.
  • Provide psychoeducation to caregivers on trauma responses, abuse dynamics, safety, and the investigative process to support informed decision-making and stability.
  • Facilitate family support or case coordination meetings for escalated or high-need cases as appropriate.
  • Provide court support, accompaniment, and case updates as needed to promote family understanding and engagement.
  • Collaborate and communicate with multidisciplinary team (MDT) partners, including law enforcement, CPS, medical, and legal professionals, to support aligned service delivery.
  • Maintain consistent engagement with families through a variety of communication methods while ensuring professionalism and appropriate boundaries.
  • Participate in and support direct service initiatives such as caregiver or youth psychoeducation groups and other CACNT programming.
  • Participate in the On-Call rotation, including after-hours, weekends, and holiday availability.
  • Maintain timely, accurate, and compliant documentation in CACNT’s electronic health record system in accordance with agency standards.
  • Participate in required training, continuing education, peer review, and supervision to support professional growth and service quality.
  • Maintain ethical standards, ensuring confidentiality, professionalism, and advocacy aligned with CACNT policies and code of ethics.
  • Other duties as assigned.


Travel and Expected Hours of Work

This position is based in the Lewisville, TX office. The candidate must be willing to work at all offices (Bridgeport, Lewisville, and Denton) when agency needs arise. Work schedule includes two evenings per week, with shifts extending up to 8:00 PM to accommodate family availability.

Education and Training

  • Bachelor’s degree in Social Work or a related field required
  • Master’s degree in Social Work or a related field preferred


Experience

A minimum of 2-years of experience in victim advocacy, case management, or trauma-related services required.
