Description:

Support individuals with disabilities and their families in leading person-centered, self-directed lives by helping them utilize available funding to access services aligned with their unique needs and life goals. Coordinate and oversee updates to service plans, budgets, and authorizations to ensure continuity and responsiveness to changing circumstances.


Position Title:
Consultation Services Coordinator

Department: Fiscal/In-Home Services

Hours: Full-time; 40 hours/week, Monday – Friday (non-exempt)

Location: Richfield/Hybrid

Compensation: $49,500 - $55,000

Bonus Eligible: Yearly

  • Can pass a background study with the Department of Human Services, a drug test, clean driving record and have reliable transportation to use for work driving purposes (e.g., in-office, on-site training locations, etc.).

How you’ll be spending your time:

  • Developing a personalized service plan in collaboration with the individual, their extended support network, and Lifeworks staff. Ensuring the plan aligns with the county and/or state Authorization for Services, and confirm receipt of a final, accurate authorization. Overseeing and facilitating any updates to the service plan, budget, and service authorization to reflect the individual's evolving needs.
  • Coordinating services for your assigned caseload by scheduling, organizing, participating in, and documenting all activities in compliance with MN 245D and 256B licensing requirements, as well as Lifeworks practices. Collaborating with the individual’s Expanded Support Team and other relevant parties to ensure service delivery aligns with expectations. Conducting meetings or site visits as required by the service line, and working closely with support staff to ensure services meet the needs and goals of the individual and their support network.
  • Auditing the service plan and/or person’s file as needed to ensure quality standards and licensing (i.e., 245D, 256B) requirements are met.
  • Conducting intakes and renewal meetings and ensuring that all required documents are complete. Providing instruction to persons and families in completing documentation and forms as requested for continued access to services and funding.
  • Providing co-supervision for employees hired to support the individual, assisting with performance management and ensuring compliance with all applicable labor laws. Supervising PCA Choice, Personal Support, and Respite staff selected by the individual and employed by Lifeworks, ensuring all services meet the standards outlined in MN 245D and 256B regulations.
  • Participating in cross-functional process improvement initiatives and projects to improve the delivery of fiscal services.
  • Staying up to date on changes to federal, state, and county statutes, licensing regulations, and funding requirements, as well as Lifeworks service updates, to ensure effective and compliant coordination of services.
  • Working collaboratively with Lifeworks staff to implement changes in statute and licensing requirements to ensure compliance across all fiscal programs.
  • Collaborating with the supervisor and finance department to ensure services are delivered within the scope of the individual's authorized funding, helping to prevent service disruptions and uncompensated care. When needed, represent Lifeworks at industry associations and government meetings to advocate for and support organizational and individual interests.
  • Following safety procedures and assist in identifying safety needs for self and others. Perform other duties as assigned.
Requirements:
  • Associates Degree in Human Services or related field with 4 years of relevant work experience required.
  • Bachelor’s Degree in Human Services or related field with 2 years of relevant work experience.
  • Meet Designated Coordinator status preferred.
  • Bilingual or multilingual preferred.
