Contract PRN Mobile Nurse - Cincinnati, OH

Avail Health is hiring a Registered Nurse (RNs) for a flexible, field-based role delivering in-person care to Medicare Advantage members across  Cincinnati, OH.

What You'll Do

• Facilitate in-person chronic disease assessments (CDAs) at long-term care facilities through virtual connection with a nurse practitioner (NP).
• Independently perform Health Risk Assessments (HRAs) and create individualized care plans, separate from NP‑led services.
• Collect patient information and complete screening tools prior to each visit
• Set up and support telehealth technology during provider visits
• Coordinate in real time with Avail Health Nurse Practitioners during assessments
• Support accurate clinical documentation and risk adjustment activities
• Manage time effectively across multiple facilities and residents within your region

Compensation

• $50.00/hour (1099 independent contractor)
• $35.00/hour training rate (compliance, technology, and clinical documentation training)
• Mileage reimbursement: $0.72/mile for distances over 30 miles/day, mileage over 200 a day must be preapproved

Schedule

• Shifts will be within Monday – Friday , 8:00 AM – 5:00 PM (does not include drive time to/from facilities)
• Contract til June 30, with potential for extension.
• Can Start As Soon As Possible

What You Need

• Current, active, and unencumbered RN license in OH
• 1+ years of clinical experience working with older adults and/or chronic disease populations
• SNF, Geriatrics, Mobile, and Long-term care setting experience preferred
• Strong communication skills and comfort working within interdisciplinary care teams

Pay: $50.00 per hour
