Duties:
-Provide case management services to individuals in need
- Conduct assessments to determine clients' needs and develop individualized service plans
- Coordinate and monitor the delivery of services to ensure client needs are met
- Advocate for clients and connect them with appropriate resources and support systems
- Assist clients in accessing healthcare, childcare, workforce development, and other essential services
- Utilize mentoring and motivational interviewing techniques to empower clients and promote self-sufficiency
- Supervise and monitor client progress, making adjustments to Treatment Plans as needed
- Maintain accurate and up-to-date case records
Experience:
- 2 years experience with a CDCA or Higher
- Experience working with diverse populations, including individuals involved in child welfare, Medicare recipients, and Section 8 housing participants
- Knowledge of community resources and ability to make appropriate referrals
- Strong communication skills, both written and verbal
- Ability to work independently and as part of a team
- Excellent organizational skills and attention to detail
