We are seeking a skilled Counselor/Case Manager to join our team in Monmouth County. In this role, you will play a vital part in supporting our clients' recovery journeys by coordinating care, providing counseling, and ensuring access to necessary resources. Responsibilities

• Develop and implement individualized care plans for clients
• Provide counseling and facilitate therapeutic groups.
• Facilitate access to community resources and services
• Collaborate with multidisciplinary teams to enhance client outcomes
• Document client progress and maintain accurate records.
• Qualifications
• Bachelor's degree in Social Work, Psychology, or related field
