Position Overview:
Kings View is seeking a Crisis Case Manager II to join our Mobile Crisis Response Team (MCRT). This role provides field-based crisis intervention and care coordination services to individuals experiencing mental health crises.
Working with minimal supervision, this position responds to non-life-threatening crisis calls, conducts assessments, coordinates care, and supports individuals in stabilizing within the community. The Crisis Case Manager II also serves as a lead resource for Crisis Case Manager I staff.
Key Responsibilities:
Crisis Response & Care Coordination:
- Respond to mobile crisis calls and provide non-clinical care coordination
- Conduct intake assessments and evaluate client needs
- Develop post-crisis engagement and stabilization plans
- Provide follow-up support within 72 hours
- Assist clients in accessing services and community resources
Client Support & Advocacy:
- Support individuals with SMI and SUD
- Provide linkage to community resources and treatment services
- Accompany clients for warm hand-offs when needed
- Assist clients in navigating referrals
Documentation & Compliance:
- Complete documentation within required timeframes
- Maintain accurate records in systems such as SmartCare and OnePoint
- Ensure compliance with COA, HIPAA, CCBHC, and CalAIM standards
Team Collaboration & Leadership:
- Provide guidance and support to Crisis Case Manager I staff
- Communicate effectively with supervisors and team members
- Collaborate with community partners
Crisis & Safety:
- Utilize de-escalation techniques and crisis intervention strategies
- Respond appropriately to high-risk situations
- Follow safety protocols and coordinate with emergency services
General Duties:
- Work flexible schedules, including evenings and weekends
- Perform other duties as assigned
What You’ll Bring:Required:- Associate Degree in Social Work, Psychology, Human Services, or related field
- Four (4) years of relevant experience (or equivalent)
- Current associate-level licensure (ASW, AMFT, or APCC)
- National Provider Identifier (NPI) or ability to obtain
- Valid California Driver’s License (Class C)
Preferred Qualifications:- Bachelor’s Degree
- Substance use experience
- Bilingual English/Spanish
