Provide compassionate, high-quality support to seniors, individuals with disabilities, and their caregivers by connecting them to essential community resources and services. This role plays a critical part in helping individuals navigate available programs and improve their quality of life.
Key Responsibilities
Client Support & Helpline Services
- Answer incoming calls in a fast-paced helpline environment
- Listen to client needs and provide appropriate resources and referrals
- Assist with programs such as Medicaid, Medicare, long-term care, and other community services
- Ensure all interactions are handled with empathy, professionalism, and confidentiality
Resource Coordination & Referrals
- Connect individuals with local service providers and agencies
- Assist clients in understanding and accessing available benefits and services
- Conduct follow-up calls to ensure services were received
Documentation & Data Entry
- Accurately collect and enter client information into a database system
- Maintain detailed and accurate records of services provided
- Respond to emails, voicemails, and written inquiries promptly
Community Engagement
- Collaborate with local organizations and service providers
- Participate in community outreach events such as health fairs and resource events
- Stay informed on available programs and community resources
Qualifications
- High School Diploma or GED required
- Minimum 2 years of experience in customer service, call center, social services, or related field
- Experience working with seniors, individuals with disabilities, or community programs is a plus

Additional Requirements

Pay: $16.00 - $20.00 per hour
