Bienvivir All-Inclusive Senior Health (“Bienvivir”) is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly (“PACE”).
PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

Bienvivir is currently accepting applications for the following position:
GENERAL DESCRIPTION:
The Daycare Escort is responsible for accompanying participants to medical specialty appointments, overseeing the safety of the participant during appointments, and for assisting with ADLs as needed while at the appointment. When not assigned to Escort duty, the position will assist in the center adult day care as assigned by the Floor Manager.
RESPONSIBILITIES:
1. Be available to attend scheduled appointments or other unscheduled (STAT) appointments designated by the Floor Manager.
2. Provide personal care needed by participants attending appointments (transfers, toileting, ambulating, etc.).
3. Communicate all necessary appointment-related information to the Floor Manager and/or Clinic Participant Manager involved with the coordination of participant care.
4. Maintain a folder that includes a personal list of the employee’s weekly participantscheduled appointments that may be modified as required.
5. Clock in/out on time at assigned centers. Will immediately report any time-keeping issues to Floor Manager.
6. Ensure participants have their snack/lunches identified by the Floor Manager and verified on the Appointment Matrix no later than the day of the appointment.
7. Attend monthly Center Staff Meetings, annual Skills Fair and other mandatory meetings/trainings as scheduled.
8. Maintain all necessary equipment and supplies (gloves, gait belt, diapers, CPR mask, appointment paperwork, etc.) needed to carry out appointment Escort duties.
9. Report to Floor Manager daily for assignments and drive from center to the participant’s appointment(s) in own vehicle.
10. Contact Clinic Participant Manager (at the center where the participant attends) and Floor Manager and if necessary activate EMS, regarding participant emergencies encountered during appointments.
11. When not assigned to Escort duties, will assist center floor operations, within their training and scope, as assigned by Floor Manager. Examples of duties may include, but not limited to, ambulating participants requiring assistance, supply inventory, assisting participants with ADLs, providing assistance to Recreation Department, handing out food trays, etc.
12. Other duties as assigned by supervisor.
QUALIFICATIONS / REQUIREMENTS:
1. Must be at least 18 years of age or older.
2. High School Diploma or equivalent required.
3. Certified Nursing Assistant in the State of Texas preferred.
4. Must have minimum one year experience caring for the frail elderly.
5. Must be CPR certified and must have a current Food Handler’s Card.
6. Experience in delivering personal care.
7. Bilingual preferred (English / Spanish).
8. Reliable transportation with valid Texas Driver’s License and current proof of insurance.
9. Required to attend twelve hours of in-service training per year. (Monthly in-services will be provided).
10. Must attend Annual Skills Fair to ensure all position competencies are current and demonstrated.
11. Will maintain current all immunizations, as required by Bienvivir Human Resources and Infection Control policies.
KNOWLEDGE / SKILLS / ABILITES:
1. Ability to handle confidential data according to policy and procedure and HIPPA.
2. Ability to work with the elderly and participants in varying stages of dementia.
3. Must possess good time management skills.
4. Ability to understand and follow specific instructions.
5. Ability to read and write English.
6. Demonstrate the ability to effectively provide personal care needed to participants attending appointments (transfers, toileting, ambulating, etc.).
7. Demonstrate ability to follow directions, document schedule information and make written adjustments to schedule on a daily basis as required.
8. Demonstrate ability to maintain written schedules.
9. Demonstrate ability to remain focused and exercise critical thinking if participant experiences medical issues during appointments.
10. Hands-on experience in the care of physically or mentally disabled participants.
11. Understand participant, home environment, aging and family dynamics.
12. Understand relationships among participant, family members and escort.
13. Be flexible in adjusting to participants schedule regarding changes in their appointments.
14. Be able to discern participant’s moods, needs and desires.
15. Be able to control and conduct tasks in the field environment.
