Job Summary:
We are seeking a dedicated Case Manager to join our team in the Hobbs area. The ideal candidate will be responsible for providing support and advocacy to individuals with developmental disabilities, in need of various social services. This role requires a compassionate individual with strong communication and problem-solving skills.
Responsibilities:
- Conduct assessments to determine clients' needs and develop individualized service plans
-Monitor ongoing eligibility, complete home and community visits
- Coordinate and monitor services to ensure clients' needs are met effectively
- Collaborate with community resources to assist clients in accessing necessary services
- Maintain accurate and up-to-date case notes and documentation
- Facilitate and document team meetings
- Advocate for clients' rights and ensure they receive appropriate care
Qualifications:
- Bachelor's degree in Social Work or related field preferred or at least 6 years experience within DDW field.
- Strong interpersonal skills and the ability to build rapport with diverse populations
- Excellent organizational skills and attention to detail
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite
- Travel within the state required. Must be able to pass a drug screen, background check and have a clean driving record.

Pay: $31,907.36 - $38,469.08 per year
