Job Summary
We are seeking a dedicated and compassionate DDW (Developmental Disabilities Waiver) Case Manager to join our dynamic team. In this vital role, you will coordinate and oversee individualized care plans for clients with developmental disabilities, ensuring they receive the appropriate services to enhance their quality of life. Responsibilities
- Develop, implement, and monitor personalized care plans tailored to each client’s unique needs, including behavioral health, child welfare, and social services.
- Conduct comprehensive assessments to evaluate clients’ strengths, needs, and goals related to addiction counseling, crisis intervention, and mental health support.
- Coordinate services such as Medicaid, workforce development programs, and resources to promote stability and independence.
- Provide crisis management and crisis intervention when urgent situations arise, ensuring safety and appropriate responses.
- Maintain detailed documentation of client interactions, service plans, progress notes, and incident reports in compliance with agency standards.
Qualifications
- Proven experience in the DDW, social work, behavioral health, or related fields
- Strong knowledge of crisis management strategies and behavioral health practices.
- Excellent communication skills with the ability to engage clients using motivational interviewing techniques.
- Ability to handle high-pressure situations with professionalism and compassion.
Pay: From $35,000.00 per year
