Detox Nurse LVN

Position Overview

We're seeking a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to support clients through detox and early recovery — overseeing medication administration, monitoring withdrawal symptoms, and ensuring care meets the highest clinical standards. This role supports patients across three residential homes and involves frequent movement throughout the homes, including regular use of stairs. This is a 12-hour shift position.

Key Responsibilities

  • Provide safe, compassionate, clinically sound care in a residential/detox setting.
  • Administer and document medications, treatments, vitals, and assessments in the EHR.
  • Monitor withdrawal using CIWA/COWS protocols; escalate significant changes to the provider or clinical lead immediately.
  • Respond to medical and behavioral emergencies and initiate emergency protocols, including EMS activation when indicated.
  • Coordinate pharmacy communications (orders, refills, prior authorizations, reconciliation) and conduct medication and controlled-substance counts per policy.
  • Conduct admission intake, including body/contraband searches performed with professionalism and respect for patient privacy.
  • Conduct wellness and safety rounds across all three homes; maintain infection-control standards.
  • Ensure compliance with DHCS, Joint Commission, HIPAA, 42 C.F.R. Part 2, and internal protocols.
  • Collaborate with the multidisciplinary team and use verbal de-escalation in crisis situations.

Qualifications

  • Active California LVN license (BVNPT) or RN license (BRN) in good standing — required.
  • Current CPR/BLS and First Aid certification — required.
  • Current TB clearance; Health Screening Report (DHCS 5077) prior to start — required.
  • Diploma or degree from an accredited Vocational Nursing or Registered Nursing program — required.
  • Willingness to complete a background check and pre-employment drug screening — required.
  • Minimum 1 year of experience in detox, residential, behavioral health, SUD, psychiatric, or emergency care — preferred.
  • Knowledge of CIWA/COWS, withdrawal management, and EMR (Sunwave/Kipu preferred).
  • Valid California driver's license preferred; able to transport patients if needed.
  • Able to walk extensively, climb stairs throughout the shift, lift up to 50 lbs with assistance, and stay composed in a fast-paced, high-acuity environment.
