About Us:
We are a well-established GP practice in Oxford serving a patient population of 26,000 across two sites, the Manor Surgery and Kennington Health Centre. Our dedicated and multidisciplinary team is committed to delivering high-quality patient care.
We have a well-developed diabetes service led by a Diabetes Lead GP, supported by an experienced Diabetes Nurse, and we are looking to expand our team with an additional Diabetes Nurse to support the delivery of high-quality care to our patients living with diabetes.
Job Summary:
We are seeking an experienced and motivated Diabetes Nurse to join our established diabetes team. The successful candidate will work closely with our Diabetes Lead GP and existing Diabetes Nurse to deliver high-quality, evidence-based care.
The role will focus on the day-to-day clinical management of patients with diabetes, including reviews, treatment optimisation, and patient education. You will play an important role in supporting the delivery of a safe, efficient, and patient-centred diabetes service within the practice.
Key Responsibilities:
- Deliver high-quality diabetes care through routine reviews, treatment optimisation, and ongoing patient management.
- Work collaboratively with the Diabetes Lead GP and wider diabetes team to manage patients with both routine and more complex needs.
- Provide patient education and support to promote self-management and lifestyle interventions.
- Accurately document consultations, clinical decisions, and management plans in real time using EMIS.
- Contribute to the effective management of the diabetes workload, including supporting triage and follow-up where appropriate.
- Work within agreed clinical pathways and protocols in line with national and local guidance.
- Support the wider clinical team with diabetes-related queries where appropriate.
- Contribute to quality improvement initiatives and audits within the diabetes service.
- Maintain up-to-date knowledge of diabetes care through continuous professional development.
- Engage in collaborative working with local diabetes services, secondary care, and community teams.

Essential Requirements:
- Registered Nurse with a valid NMC PIN.
- Experience in managing all types of diabetes (with an emphasis on type 2) within a primary care or community setting
- Ability to work autonomously as well as part of a multidisciplinary team.
- Strong organisational skills and the ability to manage workload efficiently
- Excellent communication and interpersonal skills.
Desirable:
- Postgraduate qualification or equivalent experience in diabetes management.
- Independent prescribing qualification
- Experience in service development and quality improvement.
- Familiarity with primary care systems such as EMIS.
