Direct Care Professional

Job Summary
We are seeking a compassionate and dedicated Direct Care Professional to provide exceptional support and care to individuals with diverse needs. Duties

  • Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor residents’ health by taking vital signs, observing patient conditions, and reporting changes promptly
  • Support medication administration in accordance with care plans and HIPAA regulations to ensure privacy and safety
  • Prepare nutritious meals tailored to individual dietary needs and assist residents during meal times
  • Maintain a clean and safe environment through regular cleaning, sanitation, and organization of living spaces
  • Use specialized equipment such as Hoyer lifts or resident’s <PERSON> to safely transfer individuals with mobility challenges
  • Document care activities accurately in resident records, including progress notes and incident reports
  • Engage residents in social activities and provide companionship to promote mental well-being
  • Follow all safety protocols including CPR, First Aid, and emergency procedures to ensure resident safety at all times
  • Collaborate with team members, healthcare professionals, and family members to develop and implement personalized care plans

Requirements

  • Prior experience working in assisted living facilities, group homes, nursing homes or home & community care settings preferred
  • Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, or working with individuals with developmental disabilities or autism is highly desirable
  • Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus
  • Ability to lift heavy objects safely and perform physical tasks required for patient monitoring and transfers
  • Excellent communication skills to effectively observe residents’ needs and document care activities accurately
  • Compassionate attitude with a strong commitment to providing respectful resident support
  • Flexibility to work various shifts including evenings, weekends, or overnight hours as needed
  • Ability to maintain confidentiality in accordance with HIPAA regulations while handling sensitive information
