Direct Care Professional Case management

Job Summary
We are seeking a compassionate and energetic Direct Care Professional to provide exceptional support and care to individuals with diverse needs. In this vital role, you will assist individuals, in all five pillars of life skills activities, monitor their well-being, and promote a safe, comfortable environment. Responsibilities

• Assist individuals with case management and life skills management such as the scheduling of and, transportation to healthcare appointments when necessary
• Monitor client behavioral health by, observing for changes in condition, and reporting findings accurately in compliance with HIPAA regulations
• Assist with applications and connecting individuals with community resources
• wellness checks and home visits provide support during activities and social engagements to promote mental stimulation and emotional well-being
• Maintain a safe environment by adhering to protocol.
• Follow individualized care plans and goals developed by healthcare professionals.

Qualifications

• Prior experience working in case management direct care, human service or community health services
• Knowledge of Baltimore city and surrounding area. Good attitude and observation techniques and ability to recognize signs of distress or crisis. Critical thinking skills.
• Certification in First Aid and CPR is required; additional certifications such as HIPAA training or medication administration are a plus
• Experience working with individuals with behavioral health or developmental disabilities, care is desirable
• Strong communication skills to collaborate effectively with team members, client, families, and healthcare providers
• Compassionate attitude with a commitment to providing respectful, person-centered care that promotes independence and dignity
• Transportation is required and valid insurance required.
