Responsibilities

• Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and record vital signs such as blood pressure, pulse, and temperature to ensure health stability
• Support residents with medication administration following prescribed protocols and safety procedures
• Provide compassionate Alzheimer's and dementia care, employing memory care techniques to promote comfort and safety
• Help with meal preparation, ensuring nutritional needs are met and dietary restrictions are followed
• Maintain a clean, safe environment by performing light housekeeping tasks and adhering to infection control standards
• Use Electronic Medical Record (EMR) systems to document care activities accurately and promptly
• Operate assistive devices such as Hoyer lifts safely to support resident mobility
• Engage residents in social activities and behavioral management strategies to promote mental well-being
• Support residents' participation in community outings or transportation needs using driving skills when applicable

Requirements

• Prior experience in assisted living, nursing homes, or home & community care settings is highly desirable
• Knowledge of long-term care practices including resident’s <PERSON> (personal history) documentation is preferred
• Certification or training in first aid and CPR is advantageous
• Experience working with individuals with developmental disabilities or disabilities requiring specialized support
• Familiarity with dementia care, Alzheimer's care, memory care techniques, and behavior management strategies
• Ability to accurately monitor vital signs and assist with medication administration following safety protocols
• Strong communication skills for working collaboratively with team members, residents, and families
• Valid driver’s license for transportation duties if required for community outings or appointments
