Job Summary
We are seeking a compassionate and dedicated Direct Service Provider to join our team in delivering high-quality care and support to individuals with diverse needs. In this vital role, you will serve as a trusted caregiver, assisting clients with daily activities, promoting independence, and ensuring their safety and well-being. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Administer medication and monitor vital signs following care plans and medical instructions
- Support clients with memory care needs such as Alzheimer's disease and dementia, implementing behavior management strategies
- Operate EMR (Electronic Medical Record) systems accurately to document care activities and client progress
- Prepare nutritious meals tailored to dietary requirements and assist with feeding when necessary
- Use specialized equipment such as Hoyer lifts or resident’s <PERSON> to ensure safe transfers and mobility support
- Maintain a clean and safe environment by performing light housekeeping tasks like cleaning and laundry
- Drive clients to appointments or outings while adhering to safety protocols and HIPAA regulations
- Collaborate with healthcare professionals, social workers, and case managers to develop and follow personalized care plans
- Respond promptly to emergencies with first aid skills and conflict management techniques
Requirements
- Prior experience in assisted living, nursing home settings, or home care is highly preferred
- Knowledge of working with individuals with developmental disabilities, autism, or memory-related conditions such as Alzheimer’s or dementia
- Ability to lift heavy objects safely and perform physical tasks required for caregiving duties
- Familiarity with medical terminology, medication administration, vital signs monitoring, and HIPAA compliance
- Valid driver’s license with a clean driving record for transportation duties
- Strong communication skills for effective interaction with clients, families, and team members
- Certification in first aid or CPR is a plus; ongoing training in hospice or hospice care is advantageous
- Ability to handle multiple responsibilities efficiently while maintaining a positive attitude
