Duties
- Monitor health status by taking vital signs, observing patient behavior, and reporting changes to healthcare teams
- Administer medications following care plans and HIPAA regulations to ensure safety and compliance
- Prepare nutritious meals aligned with dietary needs and assist with meal consumption as needed
- Maintain a clean, safe environment through regular cleaning duties in accordance with health standards
- Support residents during outings or community activities to promote social engagement and independence
- Document care activities accurately in compliance with HIPAA and organizational policies
Experience
- Prior experience working in group homes, assisted living, nursing homes, or home health care settings is highly preferred
- Knowledge of patient observation techniques, dementia/dementia care, Alzheimer’s care, memory care, and hospice care is advantageous
- Familiarity with assisting individuals with developmental disabilities or autism enhances your ability to provide tailored support
- Experience with patient monitoring tools such as vital signs measurement and resident’s <PERSON> mobility aids is beneficial
- Background in social work or long-term care supports your capacity to develop comprehensive care plans
