Responsibilities
This Job is 10 hours weekly . Flexible hours / Days
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or transfer belts.
- Monitor and document patients’ vital signs, health status, and behavioral changes to ensure safety and proper care.
- Administer medications and follow prescribed care plans in compliance with HIPAA regulations and healthcare standards.
- Support clients during meal preparation, cooking, and nutritional planning to promote healthy eating habits.
- Conduct patient observation to identify needs or concerns promptly, ensuring timely intervention when necessary.
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning common areas and personal spaces.
- Collaborate with healthcare professionals, social workers, and family members to develop and implement personalized care plans tailored to each individual’s needs.
Skills
- Proven experience in group home settings, assisted living facilities, nursing homes, or home & community care environments.
- Knowledge of patient care techniques including CPR, first aid, dementia care, Alzheimer’s care, hospice care, and memory care practices.
- Ability to work effectively with individuals who have developmental disabilities or autism spectrum disorder.
- Strong observational skills for patient monitoring and patient safety assessments.
- Familiarity with medication administration procedures and HIPAA compliance standards.
- Physical stamina for heavy lifting and assisting with mobility aids such as Hoyer lifts or resident transfers.
