Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or resident’s <PERSON> lifts
• Monitor and document patients’ vital signs, medication administration, and overall health status in compliance with HIPAA regulations and care plans
• Support clients during meal preparation and cooking to promote nutritional health and independence
• Provide patient observation and monitor for changes in condition, dementia or Alzheimer’s symptoms, or behavioral challenges related to autism or other disabilities
• Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning common areas and personal spaces
• Assist with community outings or activities to foster social engagement and community integration
• Follow prescribed care plans and collaborate with healthcare professionals to ensure comprehensive patient care

Skills

• Proven experience in group home settings, assisted living facilities, nursing homes, or home & community care environments
• Knowledge of patient care techniques including assisted living procedures, hospice care basics, and long-term care practices
• Strong understanding of working with individuals with developmental disabilities, autism spectrum disorder, dementia, or memory care needs
• Certified in CPR and first aid; familiarity with medication administration and vital signs monitoring
• Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or resident’s <PERSON> lifts
• Excellent observational skills for patient monitoring and recognizing changes in condition or behavior
• Compassionate communication skills to support clients’ emotional well-being while respecting privacy laws like HIPAA
