Duties
- Assist individuals with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patient health by taking vital signs and observing changes in condition, ensuring adherence to care plans and HIPAA privacy standards
- Administer medications accurately following prescribed protocols and document all medication administration activities
- Prepare nutritious meals aligned with dietary needs and assist with meal planning and cooking to promote healthy living
- Support individuals during community outings or activities to foster social engagement and community integration
- Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning common areas and personal spaces
- Document observations, care activities, and incidents meticulously to ensure compliance with healthcare regulations and care plans
Skills
- Proven experience in group homes, assisted living facilities, nursing homes, or home & community care environments
- Knowledge of patient care techniques including assisting with ADLs, patient monitoring, and vital signs measurement
- Certifications such as First Aid, CPR, HIPAA compliance training, and medication administration are highly desirable
- Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia or Alzheimer's disease, memory care needs or hospice care is preferred
- Ability to perform heavy lifting safely using equipment like Hoyer lifts and manage physical demands of caregiving tasks
- Strong observational skills for patient safety, patient observation, and early detection of health changes
- Excellent communication skills for collaborating with team members, families, and healthcare providers
