Director, Case Management

Life With Dignity is looking for a full-time Director of Case Management. This is an existing position within our growing business based in St. Charles, IL. Our company support clients across the Chicago area, both in-office and onsite at homes and care facilities. Life With Dignity delivers advocacy and support for individuals, elderly and disabled, to meet their health, psycho-social, financial, and environmental needs. We foster a culture of integrity ,enabling our team to deliver solutions with compassion in every interaction. Our Case Management services include advocacy through Life/Care Planning, Health Care Powers of Attorney, and Guardianship. The Director of Case Management will work closely with the CEO and leadership team to deliver effective, client-focused, and profitable results in collaboration with the Estate and Property management Departments

The ideal candidate brings extensive experience serving the elderly and disabled communities across healthcare, legal, and estate/property management environments. Your high sense of duty and honesty will align with our leadership team’s commitment to clear, effective communication—both internally and externally.

You will find success when you embrace management responsibilities to ensure the Case Managers deliver on their goals and commitments effectively, on time, and in alignment with LWD’s core values. Daily responsibilities include managing the Case Managers to ensure progress on assigned cases, ensuring all details are documented within the appropriate systems and communications across all departments to meet or exceed your goals.

  • Support and supervise Case Management team as assigned to include performance assessment, mentoring and disciplinary actions, with approval of the CEO.
  • Monitor and review Case Managers(CM) to ensure resources for medical care, behavioral health services, housing support, financial assistance, or protective services are being fully utilized for clients.
  • and track progress toward established goals.
  • Ensure Case Managers successfully coordinate within the constraints of the role that LWD is designated as applies to guardianship, legal representatives and agents for other authorities.
  • Facilitate communications between internal departments to ensure timely and effective client focused results.
  • Ensure proper company procedures and documentation by team to stay compliant, protective of client needs, and profitable
  • Maintain current professional working knowledge of guardianship laws and processes.
  • Daily maintain timely, accurate and complete notes for billable time and payroll.
  • Serve as escalation and support for Case Managers’ on communication and challenges.
  • Maintain collaborative relationships for services across healthcare providers, community agencies, and internal departments.
  • Coordinate and perform quarterly performance field evaluations and ongoing case supervision to ensure adherence to LWD procedures and best possible client outcomes.
  • Participate as a part of LWD Leadership to meet or exceed company goals while embodying professionalism and LWD’s core values to serve as a role model.
  • Facilitate training and on-boarding to allow success for each direct report.
  • Schedule and facilitate regular team progress and individual 1-1 meetings to allow direct reports success and answers.

● Project / Case Management experience demonstrating high acumen for benefit assignment- disability/elderly community - Min 5 yrs

● Experience managing teams of 5 or more, Min. 3 yrs

● Experience training team members

● Experience facilitating / participating in hiring process/on-boarding

● Bachelor’s degree in social work, geriatrics, mental health or equivalency

● Medicare/Medicaid benefit knowledge

● High level of Integrity / honesty

● Compassionate

● Excellent communication – verbal and written

● Excellent experience with online solutions including but not limited to email, government and agency systems, project management, etc.

● Strong organization and follow-up skills

● Professional experience using a De-Escalation strategy

● Clean driving record, and provide proof of current vehicle insurance

Preferred, though not required:

● Experience working in fiduciary services / guardianships,

● Experience with Mental Health and Developmental Disability services and benefits.

The full-time positions compensated $74,000 - $80,000 a year, depending upon experience.

Paid time off for two weeks per year for the first 2 years.

10 paid holiday

SEP IRA with matching up to 3%
