The Opportunity

ProMedix Health is looking for a Director of Clinical Operations who is equal parts clinician and operator. This is not a bedside role - it is a leadership role for an RN who is ready to own the clinical delivery infrastructure of a growing chronic care management platform.

You will be responsible for the performance, quality, and compliance of ProMedix's care coordination function - managing care coordinators, enforcing CMS documentation standards, ensuring patients receive consistent and high-quality engagement, and building the operational systems that allow the clinical team to scale without breaking. You will work closely with leadership, the Fractional Medical Director, and the engineering and operations teams to make ProMedix's clinical programs run the way they should.

Core Responsibilities

Clinical Leadership & Care Coordination Oversight

  • Lead and manage the care coordination team - providing day-to-day supervision, performance management, coaching, and accountability
  • Ensure all CCM patient interactions meet CMS documentation requirements - 20-minute monthly minimums, care plan accuracy, and audit-ready records at all times
  • Oversee RPM patient monitoring workflows - ensuring biometric alert response, device compliance tracking (16+ active days per month), and timely escalation
  • Maintain care plan quality across the patient population - reviewing for clinical accuracy, completeness, and compliance traceability
  • Serve as the clinical escalation point for complex patient situations, care team conflicts, and provider concerns
  • Ensure all patient interactions are documented in the ProMedix platform in real time - no external spreadsheets, no undocumented encounters

Compliance & CMS Standards

  • Own CMS compliance across all CCM and RPM programs - documentation standards, billing accuracy, and regulatory readiness at all times
  • Conduct regular audits of care coordinator documentation - identify deficiencies, implement corrections, and track improvement
  • Ensure monthly CCM care plans and RPM summaries are completed, exported, and attached to each provider's EHR correctly and on time
  • Build and enforce SOPs for all clinical workflows - from patient onboarding through monthly engagement through escalation and disenrollment
  • Support preparation for CMS audits - maintain documentation standards that ensure ProMedix can withstand scrutiny at any time

Operational Management

  • Manage the enrollment-to-clinical handoff process - ensuring every newly enrolled patient is set up correctly in the clinical system and assigned to a care coordinator without delay
  • Coordinate with the enrollment and operations team on patient intake, eligibility verification, and provider scheduling
  • Oversee RPM device fulfillment coordination - ensuring patients receive devices, complete training, and achieve first readings on schedule
  • Identify and resolve clinical workflow bottlenecks - proactively, before they impact patient outcomes or billing compliance
  • Contribute to EHR buildout and clinical workflow configuration - providing clinical expertise to inform system design and documentation architecture
  • Support provider onboarding and relationship management - ensuring practices have the clinical workflows, documentation templates, and communication tools they need

Team Development & Performance

  • Recruit, onboard, and develop care coordinators as the team scales - defining the hiring profile, conducting interviews, and building a high-performance clinical team
  • Set and monitor performance expectations for care coordinators - calls per shift, enrollments supported, documentation completion rates, and patient engagement quality
  • Provide regular feedback, coaching, and corrective action as needed - maintaining team standards without exception
  • Build training materials and onboarding programs for new clinical staff
  • Execute clinical protocols established by the Fractional Medical Director - and ensure care coordinators do the same

What We Are Looking For

Required

  • Active Registered Nurse (RN) license in the state of California - in good standing
  • BSN required
  • Minimum 3 years of clinical nursing experience
  • Minimum 2 years of supervisory, leadership, or management experience in a clinical or healthcare operations setting
  • Working knowledge of CMS chronic care management requirements - CCM, RPM, or similar value-based care programs strongly preferred
  • Strong documentation discipline - experience maintaining audit-ready clinical records in a compliance-sensitive environment
  • Demonstrated ability to manage and develop a team of clinical staff
  • Strong communication skills - ability to interface confidently with providers, patients, and cross-functional leadership
  • Comfort working in a fast-paced, build-stage environment where processes are still being built and improved

Preferred

  • MSN or advanced clinical or healthcare administration degree
  • Direct experience in CCM, RPM, CoCM, or TCM program operations
  • Experience with Medicare Advantage and value-based care models
  • Familiarity with EHR platforms - Canvas, eClinicalWorks, or similar
  • Experience working in a telehealth, virtual care, or remote monitoring environment
  • Prior involvement in CMS audit preparation or compliance program management

What You Will Own

This is a role with real ownership and real accountability. The Director of Clinical Operations is not a coordinator position - it is a leadership role. From day one you will own:

  • The performance and daily management of the care coordination team
  • CMS compliance and documentation quality across all CCM and RPM programs
  • The clinical workflow standards that determine whether ProMedix can bill correctly and pass an audit
  • The escalation function for complex clinical situations that require nursing judgment
  • The build-out of clinical SOPs, training materials, and onboarding processes as the team grows

You will not own revenue cycle management, business development, financial approvals, or technology platform decisions. Your lane is clinical - and it is a critical one.

ProMedix Health offers competitive compensation and a comprehensive benefits package that provides employees the flexibility to tailor benefits according to their individual needs. Our benefits package includes, but is not limited to, paid time off, a 401(k) retirement plan, medical, dental, and vision coverage, and other voluntary benefit options. Benefits may vary based on employment status, i.e. full-time, part-time, per diem or temporary.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $90,000 to $140,000 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
