The Director of Clinical Services is the senior clinical and operational leader of Home Helpers Home Care, Durham & Chapel Hill. The Director of Clinical Services is responsible for establishing and maintaining the clinical framework of the agency, ensuring regulatory compliance, overseeing care quality and caregiver performance, directing hiring and staff development, and contributing to the operational foundation of a growing agency. The owner provides active oversight, support, and collaborative leadership throughout.

As the agency grows to a sustained 150–200 client hours per week, the administrative Agency Director responsibilities will transition to a dedicated hire, and the Clinical Services Director will focus fully on clinical oversight, quality assurance, and care leadership.

Essential Job Functions & Responsibilities

Clinical Oversight & Client Care

  • Conduct initial in-home client assessments to determine home care needs and develop individualized care plans in the client’s place of residence
  • Assign appropriate personnel to client cases based on assessed needs and caregiver qualifications
  • Help fill shifts to provide client care as a last resort if shift cannot be filled
  • Review and update care plans as client conditions or circumstances change
  • Coordinate and oversee direct and indirect client services provided by agency personnel
  • Evaluate each case through record reviews, conferences, and direct observation; guide personnel to promote effective and high-quality service delivery
  • Conduct quarterly record reviews and communicate findings and recommendations to the Owner
  • Ensure proper maintenance of client records in compliance with local, state, and federal laws; oversee accurate data entry and documentation within WellSky
  • Maintain or delegate appropriate maintenance of client-related inventory, supplies, and equipment

Caregiver Supervision & Staff Development

  • Provide day-to-day clinical supervision and guidance to caregivers and home care staff
  • Facilitate the interviewing process for agency personnel; apply clinical expertise and direct service knowledge to assess candidate qualifications; provide hiring recommendations based on interview and skills assessment
  • Develop, implement, and evaluate the orientation program for new agency personnel; conduct or delegate orientation as appropriate
  • Evaluate performance of staff personnel; conduct annual reviews of direct reports or more frequently as indicated
  • Collaborate with the Owner on personnel performance concerns, progressive discipline, and termination decisions; the Owner retains final authority on all employment actions
  • Promote and support ongoing inservice education and continuing education programs for agency staff
  • Ensure compliance with local, state, and federal laws regarding licensure and certification of agency personnel

Compliance & Regulatory Adherence

  • Ensure the agency complies with all applicable local, state, and federal laws, including NC DHSR home care regulations
  • Ensure accuracy of public information materials and activities related to clinical services
  • Take action on reports and recommendations from authorized planning or quality improvement processes
  • Support preparation for state inspections, surveys, or audits
  • Ensure scheduling and staffing practices meet regulatory requirements for appropriate supervision during all operating hours

Records, Scheduling & Statistical Data

  • Develop and maintain caregiver and client service schedules in WellSky, ensuring appropriate staffing coverage and continuity of care
  • Support maintenance of statistical records in collaboration with the Owner
  • Ensure client records are current, accurate, and compliant; conduct or delegate quarterly record reviews

Agency Operations & Administrative Support

  • Provide operational insight and suggestions to inform budget planning; budget preparation, administration, and final decisions are the Owner’s responsibility
  • Ensure accuracy of public information materials and activities
  • Provide input on operational decisions related to staffing, scheduling, and care delivery in coordination with the Owner
  • Meet with Owner as planned to review/discuss agency operations/progress and respond to communication in timely manner.

Marketing Support

  • Contribute to the development of marketing strategies and referral outreach plans, drawing on clinical knowledge and community relationships; the Owner is responsible for implementation and execution
  • Develop new and leverage existing professional relationships with referral sources, including discharge planners, physicians, social workers, and community partners, as a natural extension of clinical service delivery and client intake activities
  • Serve as a trusted, visible face of Home Helpers Home Care in the Durham and Chapel Hill community via professional interactions with clients, families, caregivers, and care partners to reflect and advance the agency’s reputation for quality, compassionate care

Work Environment & Schedule

This position involves two distinct work settings. Field-based work takes place in clients’ homes and includes initial assessments, care plan development, and periodic in-home quality visits throughout the Durham and Chapel Hill service area. Office-based attendance is expected for clinical and operational meetings, staff interviews, caregiver orientations, competency assessments, and supervisory debriefs. Administrative tasks such as documentation, care plan write-ups, statistical reporting, and WellSky data entry may be completed remotely. Schedule is flexible by mutual agreement and coordinated around client and operational needs.

Position Qualifications

Required

  • Current, unrestricted Registered Nurse (RN) license in the state of North Carolina
  • Minimum of three years of management or supervisory experience in home care, home health, hospice, or a related licensed healthcare setting
  • Strong clinical assessment, documentation, and care planning skills
  • Working knowledge of or ability to quickly learn NC home care agency regulations (10A NCAC 13J)
  • Proficiency with or demonstrated ability to learn electronic care management platforms; prior WellSky experience is a significant asset
  • Excellent communication, observation, and organizational skills
  • Self-directed with the ability to work with a high degree of autonomy while maintaining consistent communication with the Owner
  • Licensed driver with a reliable, insured vehicle
  • Satisfactory references from current or prior employers and professional peers
  • Pre-employment health clearance including negative TB skin test or CXR and any other tests required by agency policy

Preferred

  • Baccalaureate degree from an NLN-accredited nursing program or degree in health, business, or public administration
  • Prior experience building or launching a home care agency or similar healthcare program
  • Experience developing clinical policies, procedures, orientation programs, or quality improvement processes
  • Familiarity with home care billing, scheduling systems, and statistical reporting
