Job Summary:
The Director of EMR Optimization provides strategic and operational leadership in the development, implementation, optimization, and evaluation of the Home Care Home Base (HCHB) electronic medical record system and related clinical technologies.


Essential Functions:

Leadership & Strategy

  • Provide overall direction and oversight for the informatics strategy across home health and hospice programs.
  • Lead the design, implementation, and ongoing optimization of HCHB and/or other EMR platforms.
  • Serve as a change agent to promote adoption of clinical technologies and workflows that improve patient outcomes and staff efficiency.
  • Collaborate with compliance, IT, operations, and nursing leadership to align technology initiatives with clinical and business priorities.
  • Manage, mentor, and develop EMR support and informatics staff.

Clinical Systems & Process Optimization

  • Translate clinical and operational needs into system requirements and functional workflows.
  • Evaluate current and emerging technologies to improve system performance, data accuracy, and reporting capabilities.
  • Identify workflow bottlenecks and implement data-driven solutions to enhance clinician productivity and patient care.
  • Coordinate the evaluation and implementation of system enhancements, version upgrades, and integrations.

Education, Training & Support

  • Develop and oversee comprehensive training and competency programs for clinicians and staff using HCHB.
  • Provide subject matter expertise to ensure optimal use of HCHB.
  • Lead initiatives to improve user engagement and satisfaction with EMR systems.


Collaboration & Communication

  • Foster strong relationships between clinical operations and IT teams to ensure clear communication and shared goals.
  • Serve as the liaison between end users, vendors, and leadership for all informatics-related matters.
  • Prepare and present regular reports to executive leadership on informatics initiatives, performance metrics, and technology updates.


Additional Responsibilities:

  • Conforms to all applicable Agency policies and procedures.
  • Sound clinical judgement and ethical decision-making.
  • Participates actively in continuing education and in-services.
  • Maintains confidentiality of patient information and business trade practices


Knowledge / Skills / Abilities:

  • Ability to develop and maintain EMR documentation standards
  • Working knowledge of HIPAA, CMS and CHAP requirements.

Minimum Position Qualifications:

  • Education:

Bachelor's degree required; Master’s degree in Nursing Informatics, Health Informatics or Computer Science preferred

  • Experience:

5 years nursing or informatics experience, including 2 years in a leadership role.

Experience with HomeCare HomeBase system configurations, implementation and optimization.

  • License / Certification:

Driver’s license and proof of current auto liability insurance; no listing in the OIG Excluded Provider listing

Active Registered Nursing compact license required.

National certification in Informatics Nursing preferred (or obtained within one year)


Environmental Conditions:

Works under a variety of conditions in facilities and offices; ability to work flexible schedule, ability to travel locally; some exposure to unpleasant weather. Moderate noise level; moderate stress and emotional demands.


Physical Requirements: Sitting is required. Requires ability to always handle stressful situations in a calm and courteous manner. Requires working under some stressful conditions to meet deadlines and agency needs. Ability to travel via plane or car.
