Position Overview

The Incident Management Director supports the Compliance Department by overseeing internal investigations related to unusual incidents and ensuring regulatory compliance. This role works closely with employees, service recipients, families, and external stakeholders to ensure incidents are thoroughly investigated, documented, and resolved in accordance with New Jersey Department of Human Services – Division of Developmental Disabilities (DDD) requirements.

The Director will lead investigative processes, manage incident documentation, and supervise the Incident Management Specialist while ensuring the organization maintains the highest standards of safety, accountability, and compliance.

Key Responsibilities Incident Investigations

  • Conduct internal investigations related to unusual incidents in accordance with NJ DDD Division Circulars 14 and 15.
  • Interview all relevant parties including employees, service recipients, guardians, support coordinators, witnesses, and other stakeholders.
  • Gather and analyze information to determine investigative findings and recommended actions.

Incident Reporting & Documentation

  • Prepare and submit Unusual Incident Report (UIR) follow-ups in compliance with regulatory guidelines.
  • Document investigative findings including incident descriptions, interview summaries, supporting documentation, and conclusions.
  • Maintain accurate and confidential records of investigations.

Systems & Data Management

  • Review and manage documentation using internal systems including:
  • Incident reporting systems
  • Service documentation platforms (Must Know Therap)
  • File management tools and document repositories
  • Retrieve and analyze information from the New Jersey Incident Reporting and Management System (NJIRMS).

Documentation Review

  • Collect and review relevant documentation including:
  • Individual Service Plans
  • Behavior Support Plans
  • Incident reports and event logs
  • Medical records and discharge summaries
  • Employee records and training logs
  • Coordinate document retrieval with departments such as HR, Operations, Training, Clinical, Finance, and Quality Assurance.

Communication & Collaboration

  • Communicate effectively with internal teams, families, guardians, and state representatives regarding investigations and incident outcomes.
  • Respond to inquiries related to incidents and compliance matters.

Leadership & Supervision

  • Supervise and support the Incident Management Specialist.
  • Participate in supervision meetings, organizational trainings, and compliance-related meetings as required.

Workplace Safety

  • Promote a safe work environment by identifying potential risks and ensuring adherence to organizational policies and procedures.

Qualifications

  • Experience conducting investigations, incident management, compliance, or regulatory oversight.
  • Knowledge of incident reporting requirements within human services, healthcare, or developmental disability services preferred.
  • Strong interviewing, documentation, and analytical skills.
  • Ability to manage sensitive information with discretion and professionalism.
  • Strong organizational and communication skills.
  • Bachelor's Degree in Social Work or a related field. (Psychology, Criminal Justice, Public Administration, Healthcare Administration)
  • Preferred: Master's degree in the given area.

Pay: $75,000.00 - $85,000.00 per year
