Director of Nursing (DON) – Home Health Care (Part-Time Startup Leadership Role)

Job Summary:

Lucky Dove Home Health Care is seeking an experienced and highly skilled Director of Nursing (DON) to play a major leadership role in launching and building a new skilled home health agency in South Carolina.

This is not a traditional “walk into an already established system” type of role. We are specifically seeking an individual with the experience, confidence, and leadership ability to help build the clinical and operational foundation of the agency from the ground up.

The ideal candidate understands not only how to maintain a home health agency, but how to help create one properly from day one, including developing systems, structure, compliance processes, workflows, and operational standards that will support long-term success.

Key Responsibilities:

  • Assist with building the agency’s clinical and operational systems from scratch
  • Help establish the overall structure and foundation of the organization
  • Develop and refine:

Clinical workflows

Documentation processes

Policies and procedures

QAPI program

Compliance systems and standards

  • Ensure compliance with:

Medicare Conditions of Participation

State regulations

Accreditation standards

  • Assist with preparation for:

State inspections

CMS surveys

Joint Commission or similar accreditation surveys

  • Provide leadership and guidance to ownership and future clinical staff
  • Help create scalable systems that will support future agency growth

Minimum Qualifications:

  • Active Registered Nurse (RN) license
  • Previous leadership experience within a Medicare-certified Home Health Agency
  • Strong understanding of:
  • Home health operations
  • Clinical compliance
  • CMS regulations and survey process
  • Joint Commission or similar accreditation standards
  • Experience participating in or leading:
  • State inspections
  • CMS surveys
  • Accreditation preparation
  • Ability to work independently and confidently in a startup environment
  • Strong leadership, organizational, and communication skills

Preferred Experience:

  • Previous Director of Nursing, Clinical Manager, or Administrator experience in home health
  • Experience helping launch, restructure, or significantly improve a home health agency
  • OASIS and home health documentation expertise
  • Experience building systems, workflows, and operational processes from the ground up

Important Note:

This role is best suited for an experienced home health professional who is comfortable stepping into a developing organization and helping create structure, systems, and long-term operational direction from the beginning.

We are specifically looking for someone who can bring both leadership and real-world operational experience, not someone seeking a routine maintenance-style role.

Pay: $40.00 - $70.00 per hour

  • This role requires operating in a startup environment with minimal structure and making independent decisions. Are you comfortable in this type of role?

Experience:

  • Home Health Administrator/Leadership: 5 years (Required)
