Job Summary
Eagle Health is seeking a highly qualified Director of Nursing (RN) to oversee clinical operations, regulatory compliance, and care coordination for clients receiving home and community-based services. This position provides clinical leadership for programs operating under guidelines established by the Colorado Department of Public Health and Environment (CDPHE) and the Colorado Department of Health Care Policy and Financing (HCPF), including In-Home Support Services (IHSS) and Personal Care Services. The Director of Nursing is responsible for ensuring that all clinical services are delivered safely, eAectively, and in compliance with state regulations. This role involves case management, interdisciplinary care coordination, care planning, regulatory oversight, staA education, and supervisory oversight of caregivers. The Director of Nursing will also conduct clinical audits and ensure all documentation meets regulatory standards required by CDPHE, HCPF, and program partners such as Consumer Direct Colorado. The ideal candidate will be an experienced Registered Nurse with strong leadership skills, excellent regulatory knowledge, and demonstrated ability to manage high-volume caseloads within home and community-based healthcare programs.
Clinical Oversight and Case Management
Provide clinical leadership for IHSS and Personal Care services delivered through Eagle Health.
Manage and oversee a high-volume caseload exceeding 100 clients annually while ensuring all clinical documentation remains accurate, compliant, and timely.
Conduct initial assessments, ongoing evaluations, and care plan reviews for clients receiving services.
Ensure client needs are assessed holistically, including physical health, functional limitations, and support needs within the home environment.
Monitor client conditions and ensure any changes in health status, hospitalization, injury, or service needs are properly documented and reported in accordance with state regulations.
Ensure services delivered by caregivers remain within the authorized scope of services.
Care Planning and Implementation
Develop individualized care plans that comply with HCPF and CDPHE regulatory standards.
Ensure care plans clearly outline: o Approved services o Safety considerations o Functional support needs o Risk mitigation strategies
Coordinate care plan implementation with caregivers, case managers, and interdisciplinary providers.
Review and update care plans when there are changes in condition, services, providers, or living arrangements.
Ensure all care plans accurately reflect services authorized under the IHSS and Personal Care programs.
Care Coordination Across Agencies
Coordinate care between multiple providers, including: o Medicaid case management agencies o Physicians and specialists o Hospitals and rehabilitation facilities o Behavioral health providers o Community service agencies
Maintain communication with case managers and external providers to ensure continuity of care.
Ensure all services remain aligned with Medicaid waiver requirements and individual service plans.
Address service gaps or safety concerns through proactive care coordination.
Quarterly Supervisory Visits
Conduct required quarterly supervisory visits with clients receiving services.
Evaluate caregiver performance and adherence to the care plan. Assess client satisfaction, safety, and service quality.
Document supervisory visits according to CDPHE and HCPF documentation requirements.
Identify and address any compliance or safety concerns during visits.
Regulatory Compliance and Quality Assurance
Maintain agency compliance with all regulations established by the Colorado Department of Public Health and Environment and the Colorado Department of Health Care Policy and Financing.
Conduct internal compliance audits for: o Client files o Care plans o Supervisory visit documentation o Caregiver documentation o Incident reporting
Ensure all services provided fall within the approved scope of IHSS and Personal Care services.
Maintain readiness for regulatory surveys, audits, and inspections.
Develop corrective action plans when deficiencies are identified.
Staff Training and Development
Provide clinical training and competency validation for caregivers, including Personal Care Workers and Homemakers.
Educate staA on scope-of-practice requirements under IHSS and Personal Care programs.
Provide ongoing training related to: o Client safety o Infection control o Documentation requirements o Incident reporting o Changes in regulations
Ensure caregivers understand when to report: o Changes in client condition o Hospitalizations o Injuries o New services or agencies involved in care
Conduct regular staff compliance trainings and clinical refreshers.
Client and Caregiver Compliance Education
Provide training and guidance to clients and caregivers regarding program rules and service limitations.
Ensure caregivers understand what services they are authorized to perform and the boundaries of their role.
Address compliance issues promptly and provide corrective training when needed.
Experience
Knowledge and Skills
Strong working knowledge of regulations established by: o Colorado Department of Public Health and Environment o Colorado Department of Health Care Policy and Financing
Clear understanding of IHSS and Personal Care service scope and limitations.
Exceptional time management and organizational skills.
High attention to detail and accuracy in clinical documentation.
Strong communication and care coordination abilities.
Ability to independently manage complex caseloads and competing priorities.
Screening and Compliance Requirements
All candidates must be able to:
Pass a comprehensive background check.
Successfully complete a CAPS screening.
Pass a drug screening.
Complete required regulatory training through: o Consumer Direct Colorado o Colorado Department of Health Care Policy and Financing o Colorado Department of Public Health and Environment
Maintain compliance with all Medicaid and state regulatory requirements.
