Rockaway Care Center is looking for a full time Social Worker to work with our dynamic team and residents at our facility.
Responsibilities:
- Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident/
- The social worker will develop relationships with the residents and their families to ensure that their needs are provided in a timely and dignified manner
- Ensure that social services components of the MDS are completed and signed on a timely basis.
- Participate in Facility surveys (inspections) made by authorized government agencies, as necessary or as may be directed.
- Participate in discharge planning, development and implementation of social care plans and resident assessments.
- Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
- Assure that social service progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
- Educate residents and families about the implementation of Advanced Directives.
- Upon admission review required policies and procedures.
Education:
- Must possess, as a minimum, a Bachelor’s Degree in social work or psychology from an accredited school.
- Licensed Masters Social Work preferred from an accredited school.
- Experience: Must have, as a minimum, one (1) year experiences as a Social Worker in a hospital, nursing care facility, or other related medical facility, preferred

- Must be able to read, speak, write and understand the English Language.

- Must possess the willingness to work harmoniously with professional and non-professional personnel.
- Must possess patient, tact and a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents, if necessary.
- Demonstrates commitment to continuing professional development.
- Ability to recognize and later describe persons associated with nursing homes safety and security.
Working Conditions
- Works in office areas as well as throughout the nursing service area (i.e., units, resident’s rooms, etc.)
- Sits stands, bends, lifts and moves intermittently during working hours.
- Is subject to frequent interruptions.
- Is involved with residents, personnel, visitors, government agencies, under all conditions/circumstances.
- Is subject to hostile and emotionally upset residents, family members, personnel and visitors.
- Communicates with all of the interdisciplinary team including medical staff.
