Director of Social Work

Rockaway Care Center is looking for a full time Social Worker to work with our dynamic team and residents at our facility.

Responsibilities:

  • Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
  • Identifying individual social and emotional needs
  • Developing and maintaining individualized care plans
  • Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
  • Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident/
  • The social worker will develop relationships with the residents and their families to ensure that their needs are provided in a timely and dignified manner
  • Ensure that social services components of the MDS are completed and signed on a timely basis.
  • Participate in Facility surveys (inspections) made by authorized government agencies, as necessary or as may be directed.
  • Participate in discharge planning, development and implementation of social care plans and resident assessments.
  • Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
  • Assure that social service progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
  • Educate residents and families about the implementation of Advanced Directives.
  • Upon admission review required policies and procedures.

Education:

  • Must possess, as a minimum, a Bachelor’s Degree in social work or psychology from an accredited school.
  • Licensed Masters Social Work preferred from an accredited school.
  • Experience: Must have, as a minimum, one (1) year experiences as a Social Worker in a hospital, nursing care facility, or other related medical facility, preferred

Specific Requirements:

  • Working knowledge of state and federal regulations.
  • Must be able to read, speak, write and understand the English Language.
  • Must possess the ability to make independent decisions when circumstances warrant such actions.
  • Must possess the ability to deal tactfully with personnel, residents and family members.
  • Must possess the willingness to work harmoniously with professional and non-professional personnel.
  • Must possess patient, tact and a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents, if necessary.
  • Demonstrates commitment to continuing professional development.
  • Ability to recognize and later describe persons associated with nursing homes safety and security.

Working Conditions

  • Works in office areas as well as throughout the nursing service area (i.e., units, resident’s rooms, etc.)
  • Sits stands, bends, lifts and moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is involved with residents, personnel, visitors, government agencies, under all conditions/circumstances.
  • Is subject to hostile and emotionally upset residents, family members, personnel and visitors.
  • Communicates with all of the interdisciplinary team including medical staff.
