Job Purpose
To direct and oversee daily operation of the Healthcare Home team, coordinate activities and ensure that the program achieves FCC’s mission of Person-Centered Recovery and Wellness, and that it operates in line with the core values of Integrity, Empowerment, Excellence, Compassion, and Commitment.
Qualifications

Registered Nurse or Licensed Practical Nurse with current Missouri license.

Five years primary care experience in Nursing and experience with healthcare technology preferred.

Meet the minimum requirements to be clinically privileged by FCC Behavioral Health.
Core Job Duties
-
Assists the CCBHO in developing/enhancing wellness initiatives, staff/consumer education, health fairs, and screenings.

Responsible for management of HIT tool initiatives on a monthly basis using data from CareManager and communicating the gap needs to the Nurse Care Managers monthly for interventions.

Supervises nursing staff and has knowledge of the principles and practices of effective supervision and performance.

Assure that all nursing licenses, certificates, and credentials are current with no lapses.
- Assure that all staff are fully trained and attend all required trainings, meetings, & webinars, and that appropriate documentation is maintained.

Assure that all monthly assigned HCH data reporting due to the Missouri Dept of BH is done in a timely manner.

Will serve as part-time Nurse Care Manager, providing direct care management and nursing support as part of assigned caseload.

Monitor critical chronic disease indicators and population health reports using CareManager.

Identify high-risk consumers through health screenings, assessments, and ongoing evaluation.

Develop individualized recovery and wellness care plans for assigned persons served.

Complete and track required assessments, screenings, and documentation in compliance with agency standards.

Completes monthly review of pertinent information in CareManager to address any concerns with Nurse Care Manager.

Provides consultation with the physician leadership and Nurse Care Managers regarding Healthcare Home functions or client specific concerns, as needed.

Assist in development of the individualized recovery care plan for assigned person(s) served.

Has knowledge of principles and practice of state, federal, and local laws regarding Healthcare Home and nursing regulations and legalities.

Adherence to all applicable evidence-based practice models.

Orally communicate information effectively and accurately.

Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.
- Abide by program and agency policies and procedures.
