Position: Domestic Violence Shelter Case Manager
Hours: Full-time: 40 hours/week
Status: Exempt
Salary: $45,000/yr

POSITION SUMMARY
Position is responsible for delivery of direct services to clients in the Domestic Violence Shelter by providing case management, support, advocacy, education, crisis intervention, and community referrals.
ESSENTIAL DUTIES / RESPONSIBILITIES
- Meet with clients upon entry into Domestic Violence Shelter to create case management plans
- Provide information and education as determined by case management plan
- Maintain ongoing contact with shelter clients to work on case management plans and planned discharge from shelter
- Assist clients with orientation to shelter; provide extension notification and explain extension policy
- Assist clients with referrals and interaction with other agencies
- Attend weekly Front Door meetings to gather referrals and advocate for client’s housing needs
- Accompany and transport clients to appointments as necessary
- Adjust weekly schedule and working hours to meet with clients as necessary
- Network with community services for the provision of quality services for clients
- Participate in community work groups and provide partners with information on NFHC programs
- Create monthly DV Shelter educational group calendar; assist staff in facilitating groups for clients
- Update DV Shelter resources and update community resource notices for clients
- Update Needs Assessment list weekly
- Monitor and assist in notification of ineligibility within the program based on policy and procedures
- Assist advocates on shift, including debriefing and crisis intervention
- Assist with training new staff and interns
- Assist with interviewing new hire and intern candidates as needed
- Maintain adequate records and document interactions with clients, including inputting of notes and referrals into the DV Shelter database
- Assist with monitoring client records in DV Shelter database including accurate input of client data, intakes information, and closing out clients after discharge from shelter
- File review of closed/exited client files to include oversight of confidentiality requirements
- Assist with monthly, quarterly, and annual reports as assigned
- Assist in maintaining health and safety of shelter environment, including monitoring and cleaning of rooms and common spaces
- Assist with on-call rotation and shift coverage
- Lead client updates at weekly staff meetings and clinical meetings
- Participate in all assigned staff meetings and scheduled trainings
- Collaborate with DVS staff team and Clinical Support regarding client care, goals & discharge
- Other duties as may be assigned
KNOWLEDGE, SKILLS, AND ABILITIES
- Understanding of trauma informed care in service delivery, crisis intervention skills, group facilitation, and victim advocacy to assist survivors of domestic violence, family violence, sexual assault, child abuse, and human trafficking
- Knowledge of community-based services and referrals for clients
- Understanding of protecting client rights and maintaining confidentiality of client records and client information
- Flexibility with assignment of tasks based on the needs of the shelter
- Must be willing and able to perform First Aid and/or CPR procedures if necessary
MINIMUM QUALIFICATIONS
- Must be at least 21 years of age
- Bachelor degree, or
- Associate degree with minimum of 1 year experience in field related to behavioral health, social services, and/or human services
- Any equivalent combination of experience, training, or education approved by Executive Director
PREFERRED QUALIFICATIONS
- Experience in case management and/or leadership positions
REQUIRED CERTIFICATIONS
- Valid driver license
- Diploma or official transcripts
- Successful annual TB test (can be completed upon hire)
- Successful biannual physical exam (can be completed upon hire)
- Adult, child, and infant First Aid/CPR (can be completed upon hire)
- Fingerprint Clearance Card (can be completed upon hire)
- Successful background check(s) (will be completed by NFHC upon hire)
- Any other certification or acknowledgment as may be required by the agency, licensure, or funders
WORKING CONDITIONS / PHYSICAL REQUIREMENTS
- Position requires periods of desk time and periods of mobility
- Position involves frequent interaction with clients living in shelter, individuals with mental health challenges, and abuse survivors, both adult and child
- Position may require lifting/carrying up to 50 lbs
- Position may require kneeling, reaching, and stooping
- Position requires fluent and effective communication skills with diverse age groups and populations
