The DREAM Enrichment Case Manger is responsible for facilitating and implementing an individualized and outcome-based planning process for each person receiving services and for assisting the Director of Waivered Services to ensure that outcomes are being implemented correctly and that data being collected is accurate.

Essential Functions

1. Facilitate and implement personal planning processes

2. Write Individual Programs

3. Supervise Staff

4. Complete all 245D paperwork

$23/hour

For over 45 years, Functional Industries has assisted individuals with disabilities by successfully pairing them with local employment opportunities through employee development, job placement and retention services and helping them work on a variety of independent living skills. We teach/train meal preparation, personal hygiene, shopping, cleaning, home organization, community safety and/or learning community transportation. If you are interested in helping others in everyday living skills, we want you on our team. We have full time or part time positions available, and we offer flexibility in scheduling.
