DS- Case Manager

Job Summary
We are seeking a dedicated and compassionate DS- Case Manager to join our dynamic team. In this vital role, you will coordinate and deliver comprehensive case management services to individuals navigating behavioral health challenges, including addiction and mental health issues. You will be responsible for organizing, facilitating, and documenting the service planning for individuals with Developmental Disabilities as well as working collaboratively with other community resources to ensure the quality of service provision.

Qualifications:

Responsibilities

• Associate degree or equivalent required, Bachelor’s Degree preferred
• Two-three years of relevant field experience preferred
• Excellent oral and written communication skills
• Ability to utilize technology provided to perform requirements of the job
• Hold a valid driver’s license and proof of vehicle liability coverage.
• Must demonstrate excellent multitasking abilities.

Pay: From $20.00 per hour
