Early Intervention SERVICE COORDINATOR Bilingual Uzbek

Job Overview:

Bright Start PT & PTA, PLLC is a NYS Early Intervention agency specializing in providing Early Intervention services to children from birth to three (3) years of age in all 5 NYC boroughs as well as Nassau and Suffolk Counties on Long Island. We are currently seeking dedicated Uzbek speaking Service Coordinator to assist parents in developing of plans and strategies to meet the needs of their children and family.

Responsibilities:

- to ensure that parents are informed about the Early Intervention Program (EIP) and their rights under the program

- to coordinate all aspects of the screening and evaluation process to determine a child's eligibility for the Early Intervention Program.

- to discuss the Individual Family Service Plan ( IFSP) process with the parents of children determined to be eligible for early intervention services

- to participate in the meetings to develop the IFSPs

- to secure and arrange for providers to deliver Early Intervention Program services

- to monitor the delivery of early intervention services in accordance with the IFSP

- to maintain proper documentation on families and children in accordance with EI requirements

Requirements

• Fluency in Uzbek and English, with excellent verbal and written communication skills in both languages.
• Bachelor's or Master's degree in a health or human service field including psychology, social work, special education, early childhood development, or other related licensed healthcare professional
• One year of service coordination experience and an Associate's Degree in a health or human service field
• Two years of experience in service coordination activities
• Strong organizational and communication skills, with the ability to effectively manage multiple tasks and prioritize responsibilities
• Ability to work independently and as part of a team

Pay: $55,000.00 - $75,000.00 per year
