Position Summary
BHB is hiring three (3) ECM Case Managers to join our care team. The Case Manager serves as the Lead Care Manager (LCM) for Medi-Cal members enrolled in BHB's Enhanced Care Management program. This is a hands-on, community-facing role that combines in-office coordination with regular field-based outreach, meeting members where they are, whether in their homes, at clinical appointments, at shelters, or in other community settings. The Case Manager develops individualized care plans, coordinates services across systems, and serves as a consistent and trusted point of contact for each member on their caseload.
This position requires travel across BHB's contracted service counties, which include Monterey, Santa Cruz, Merced, and San Benito. Candidates must have reliable transportation and a willingness to travel as member needs require.
Key Responsibilities
- Conduct outreach, engagement, and enrollment of eligible Medi-Cal members into the ECM program, including those experiencing homelessness, chronic illness, behavioral health challenges, or housing instability
- Complete comprehensive assessments to identify each member's medical, behavioral health, social, and housing needs
- Develop, implement, and regularly update individualized care plans in alignment with DHCS ECM program requirements
- Serve as the primary point of contact and advocate for members by facilitating warm referrals and linkages to medical providers, specialists, behavioral health services, community supports, and housing resources
- Conduct regular in-person and telephonic check-ins with members and perform field visits across contracted service counties as needed to meet members where they are
- Coordinate care transitions including hospital discharges, emergency department follow-up, and linkages to post-acute services
- Maintain accurate and timely documentation of all member interactions, assessments, care plans, and referrals in compliance with DHCS ECM documentation standards
- Collaborate with Managed Care Plan (MCP) representatives, primary care providers, and community partners as part of an interdisciplinary care team
- Participate in team meetings, case conferences, trainings, and quality improvement activities
Minimum Qualifications
- High school diploma or GED required; an Associate's or Bachelor's degree in Social Work, Human Services, Public Health, Psychology, or a related field is strongly preferred
- Minimum of 1 year of experience in case management, community health, social services, care coordination, or a related field serving vulnerable or high-need populations
- Knowledge of or experience with Medi-Cal, CalAIM, ECM, or Community Supports programs is highly desirable
- Strong communication, organizational, and interpersonal skills
- Ability to work independently in the field and manage a caseload with competing priorities
- Valid California driver's license, access to a reliable vehicle, and willingness to travel across Monterey, Santa Cruz, Merced, and San Benito Counties
- Comfort working with individuals experiencing homelessness, substance use challenges, mental health conditions, and complex social needs
Preferred Qualifications
- Lived experience with the populations BHB serves, including housing instability, recovery, or navigating complex systems
- Peer Support Specialist, Community Health Worker (CHW), or a similar certification
- Experience working in a faith-based, nonprofit, or community-based organization
- Bilingual in English and Spanish strongly preferred
- Familiarity with local resources and service systems in Monterey County and surrounding counties
Compensation and Benefits
- Annual salary of $65,000 - $31.25 per hour for standard FT 40 hour work week
