EHE Engagement Clinic Lead

Organizational Summary:

Prevention Point Philadelphia (PPP) is a multi-service public health organization dedicated to protecting the health and welfare of the homeless, drug users, sex workers, and the uninsured, in the Kensington neighborhood of Philadelphia. Using a harm reduction approach, PPP offers culturally sensitive, non-judgmental services to address the health and social service needs of drug users and sex workers in Philadelphia. PPP meets people where they are, providing information and access to resources with dignity and respect. PPP promotes harm reduction through mobile medical care, sterile syringe exchange, referrals to social services, and comprehensive prevention case management services.

Program Overview:

The Sana Clinic delivers low-barrier HIV care in a walk-in clinic structure. The program continues to expand outreach, engagement, and care coordination efforts to improve linkage to and retention in care. Serving individuals living with HIV—particularly those who use drugs, are new to recovery, or are experiencing unstable housing—the clinic operates as a “one-stop-shop” model that integrates medical, behavioral health, and social services. The EHE Engagement Clinic Lead works with a multidisciplinary team to support clinic operations and expand access to high-quality, low-barrier care.

Position Overview

The EHE Engagement Clinic Lead plays a central role in ensuring smooth clinic operations and delivering patient-centered, low-barrier care within the Sana Clinic. This role is responsible for coordinating clinic flow, supporting patient engagement and retention, and managing key operational systems, including medication tracking, documentation, and care coordination, in alignment with Ending the HIV Epidemic (EHE) goals.

Sana Clinic operates on a walk-in, low-barrier model, requiring flexibility, strong real-time decision-making, and the ability to navigate a dynamic, fast-paced environment while minimizing barriers to care.

A primary focus of this position is identifying, engaging, and re-engaging patients—particularly those who are lost to care or at risk of falling out of care—while ensuring a “red carpet” experience that promotes trust, continuity of care, and ongoing engagement. The Clinic Lead also supports continuity of care by collaborating with case managers and assisting with urgent patient needs and care coordination tasks during clinic hours.

This role serves as a key bridge between medical providers, case managers, outreach staff, and patients, helping to ensure a coordinated, responsive, and team-based approach to care.

Responsibilities

Clinic Operations

  • Lead day-to-day clinic flow in low-barrier, walk-in environment to ensure an efficient, patient-centered experience
  • Triage patients upon arrival, including taking vitals and supporting intake process.
  • Communicate and communicate real-time patient flow, including scheduling adjustments and patient updates.
  • Assist with patient navigation during visits (paperwork, labs, connection to case manager, etc.)
  • Support providers and case managers, including during the new diagnosis (“fast track”) protocol.

Medication & Lab Coordination

  • Track and coordinate medication orders and deliveries as well as manage inventory; troubleshoot delays with pharmacies and insurance providers.
  • Maintain and update medication logs and spreadsheets; ensure proper storage of medication.
  • Coordinate labs, including specimen handling and results tracking.
  • Complete and track pre-authorizations.

Documentation & Program Coordination

  • Maintain accurate patient records in EMR and assist with data input in Careware.
  • Assist in and ensure timely completion of required documentation (e.g. consents, screeners, labs, visit frequency); flag any gaps.
  • Develop agendas for weekly team meetings and support team coordination.
  • Manage medical record requests.

Patient Engagement & Outreach

  • Identify and schedule patients due for visits and labs
  • Identify and support re-engagement of patients lost to care or are at risk of being lost to care
  • Conduct community-based outreach (including street-based) with re-engagement specialist
  • Conduct outreach using EMRs, external systems (e.g. docket searches), interdepartmental communication, and collateral contacts
  • Facilitate warm handoffs and reduce barriers to timely linkage and ongoing care.

Perform other duties as assigned

Qualifications

  • Minimum of one year working with people who are living with HIV or complex medical and social needs.
  • Strong organizational skills and attention to detail
  • Ability to manage multiple priorities in a fast-paced environment
  • Strong communication and interpersonal skills across diverse populations
  • Ability to use a harm reduction approach with people who use drugs and engage in sex work.
  • Ability to work independently and collaboratively as part of a team.
  • Fluency in English. Spanish a plus.
  • Dynamic, creative, flexible, able to learn and adapt in a changing environment.
  • Proficiency in Microsoft Office, experience with electronic medical record systems a plus.
  • Belief in promoting the health and dignity of individuals who use drugs through a harm-reduction lens.
  • Strong computer and professional writing skills

Other Requirements:

  • This position is full-time (40 hours per week) and in-person.
  • This position includes the normal physical demands of a community-based health program, including lifting/moving up to 25lbs, bending, and carrying.
  • Other physical requirements include walking and standing for long periods and sitting for up to 30 minutes.
  • This position requires travel to outreach locations within Philadelphia and the ability to work out-of-doors for up to 3 hours

This position is exempt from the overtime provisions of the Fair Labor Standards Act

Job Type: Full-time

Pay: From $48,000.00 per year
